Employee Coverage. For employee dental coverage, the Employer contributes an amount equal to the lesser of ninety percent (90%) of the employee premium of the State Dental Plan, or the actual employee premium of the dental plan chosen by the employee. However, for calendar years beginning January 1, 2019, the minimum employee contribution shall be thirteen dollars and fifty cents ($13.50) per month.
Employee Coverage. For employee health coverage for the 2022 and 2023 plan years, the Employer contributes an amount equal to ninety-five percent (95%) of the employee-only premium of the Minnesota Advantage Health Plan (Advantage).
Employee Coverage. The employee’s cost for employee-only coverage will be twelve percent (12%) of the employee-only rate for the Base Dental Plan. If the employee chooses a plan other than the Base Dental Plan, the employee’s cost will be the standard employee rate established for that plan (i.e., the rate applicable where it has not been modified to be a zone’s Base Dental Plan). The employer shall pay the rate over and above the employee’s cost for the Base Dental Plan.
Employee Coverage. The employee’s cost for employee-only coverage will be thirteen percent (13%) of the employee-only rate for the Base Medical Plan. If the employee chooses a plan other than the Base Medical Plan, the employee’s cost will be the standard employee rate established for that plan (i.e. the rate applicable where it has not been modified to be a zone’s Base Medical Plan.) The employer shall pay the rate over and above the employee’s cost for the Base Medical Plan.
Employee Coverage. For employee health coverage, the Employer contributes an amount equal to one hundred (100) percent of the employee-only premium of the Minnesota Advantage Health Plan (Advantage).
Employee Coverage. The BOARD shall provide and fully fund (100% District paid) comprehensive health insurance as defined in the SPD for all eligible EMPLOYEES covered under this section, excluding those grandfathered employees referred to in Section 12.071.
Employee Coverage. Eligible Employees Subject to the Conditions of Eligibility set forth below, and to all of the other conditions of the Contract, all of the Contractholder's Employees[ who are in an eligible class and] who live, work or reside in the Service Area will be eligible if the Employees are [Actively at Work] Full-Time Employees. [In certain situations, the Actively at Work requirement will not apply. If an Employee is not Actively at Work due to a Health Status-Related Factor, the Employee will nevertheless be considered an Eligible Employee. In addition, refer to the Exception below.]
Employee Coverage. The BOARD shall provide and fully fund 100% District paid premium payments of comprehensive health insurance as defined in the Plan for all eligible EMPLOYEES covered under this section.
Employee Coverage. 1. Two-Employee/Two-Person Coverage. Each employee covered under two-employee/two-person coverage shall pay the individual employee’s cost for single coverage.
