Medical Insurance Benefit. Any priest receiving a benefit under the retirement program of the Diocese shall also be eligible, at Diocesan expense, for coverage under the medical and dental insurance provided for priests of the Diocese. Priests who are receiving retirement benefits and who reside outside of the United States, e.g., in Europe, shall have the option to have the above-stated coverage or to receive, in lieu of coverage, an amount equal to that which would have been paid for coverage. This amount will be added to and paid along with the monthly retirement benefit.

(See Appendix Cii)  **On July 1, 2021, the district’s audited fund balance was greater than $2,100,000, the district will provide an additional benefit of $10 per month toward health insurance costs for those receiving health benefits. Pharmacy is a $250 deductible per year, with a $10/20/30/50/20%/30% co-pay. Maximum OOP for pharmacy is $1,000. Employee will have $67.03 to be utilized in a Cafeteria Plan to purchase other insur- ance products. (See Appendix Cii)
Medical Insurance Benefit a. The District shall provide reimbursement for up to two co-pay fees for unit members and covered individuals on a HSA-Minimum Value, HSA-B or HSA-A medical plan who choose to use the MDLive telemedicine program (up to a maximum reimbursement of $40 per use, $80 total; under no circumstance shall the District reimbursement exceed $40 per use or $80 total).
Medical Insurance Benefit. The District will contribute an amount as determined by the Public Employees Benefits Board (PEBB) annually for benefits in calendar year 2014 and thereafter. The District shall deduct any employee contributions necessary to fully fund PEBB coverage. As determined by the PEBB, this insurance may include dental, life and long-term disability insurance coverage.

Medical Insurance Benefit. Employer shall provide a Health Plan/Schedule C for each employee desiring such coverage for him/herself and his/her dependents, if any. Moreover, the employer’s monthly insurance contribution shall increase in year one of the contract to match stipend received by all other City employees. In subsequent years, the stipend shall increase by the percentage of the rate increase. The value of the stipend shall not be less than the stipend offered to all other City employees. As of January 1, 2020 the City employee stipend is $848 per employee and at the beginning of this contract term on July 1, 2020, the stipend will increase for IAFF 630 members to at least this amount or higher depending on the allocation of the city employee stipend which will be determined in or around April, 2020. The City will pay the monthly contribution, toward the health insurance coverage. The employee shall decide plan selection, within the City’s provider. Additional funds, not used in premiums, will be deposited into a Flex or HSA account for full time employees and a pro-rated amount for part time employees subject to the following limitations:
Medical Insurance Benefit. All priests receiving benefit under this policy will as part of the retirement benefit be provided medical and dental insurance under the group policy for the priests of the Diocese. Only priests who reside outside of the United States will have the option to receive this benefit by way of an increase in benefit payment.
