On receipt of such request and after due scrutiny, GIITL will issue Authority Letter and/or Authorization Letter (AL) specifying beneficiary’s entitlement of available benefits. The AL will be either faxed to the NSP or hand delivered by the beneficiary and produced at the time of admission. The Provider will not admit any GIITL beneficiary without AL except in case of emergency. The Authorization from GIITL will clearly mention in BOLD prints that all reports and films will be sent to GIITL and not be handed over to the beneficiary. The beneficiary will be treated as a credit patient on GIITL Account. The NSP shall allow the credit facility up to the maximum limit as mentioned in the Authority Letter issued by XXXXX. The expenses incurred in excess of the limit allowed in the AL shall not be payable by GIITL and the NSP shall recover such expenses directly from the beneficiary before discharge. In case of emergency, when the beneficiary requires emergency admission the NSP shall admit the beneficiary on the basis of Photo Identity card issued by XXXXX and arrange to send the information through Pre-Authorization Form duly completed within 24 hrs. Procedure for Admission. After the Effective Date, a Person (other than an existing Partner) who makes a Capital Contribution to the Partnership in accordance with this Agreement shall be admitted to the Partnership as an additional Limited Partner only upon furnishing to the General Partner (i) evidence of acceptance, in form satisfactory to the General Partner, of all of the terms and conditions of this Agreement, including, without limitation, the power of attorney granted in Section 10.5 hereof and (ii) such other documents or instruments as may be required in the discretion of the General Partner in order to effect such Person's admission as an additional Limited Partner. Notwithstanding anything to the contrary in this Section 14.1, no Person shall be admitted as an additional Limited Partner without the consent of the General Partner, which consent may be given or withheld in the General Partner's sole and absolute discretion. The admission of any Person as an additional Limited Partner shall become effective on the date upon which the name of such Person is recorded on the books and records of the Partnership, following the consent of the General Partner to such admission.

He can help by producing any of the documents like, Aadhaar card, Voters ID, Driving license, Address Proof, Telephone number, Mobile Number, Bank name, address, Account number, Branch code, IFCS Code, MICR Code, to establish his identity and to facilitate Electronic Claim Settlement by the TPA. The MEDICAL PROVIDER should send a Request for Authorization Letter (RAL) for treating the Beneficiary in IRDAI format. On receipt of such request and after scrutiny, discussion with the treating Physician or Surgeon, “Anyuta Insurance TPA in Healthcare” (A – ITPA-H) will issue the Authorization Letter (AL) within 48 hours. The AL will be e mailed, faxed, posted, courier to the Medical Provider or hand delivered by the beneficiary at the time of admission. The Provider will not admit any beneficiary without sending the RAL except in the case of Medical Emergency. All original reports/ Xxxxx duly signed by the beneficiary should be sent to M/s “Anyuta Insurance TPA in Healthcare” (A – ITPA-H) address at Bangalore. The beneficiary should be explained by the Medical Provider in detail, in the language that he and his immediate relatives understand, about his Health (medical or surgical) conditions, the need for Specialists Care over Family Physicians Care, the need for Hospitalised Care over the Out – Patient or Domiciliary Care, the options of treatments available (conservative or surgical) with health and cost benefits, the best line of treatment in his case, the reason for adopting such line of treatment, the outcome of such treatment, the need for each of the laboratory investigation, invasive or non invasive procedures, the number of days of admission, the reason for each days admission, follow up procedures, costs incurred in each event in the hospital and after discharge, reason for such costs, payment module, Co – payment if any, the reason for such payments, the anticipated outcome of the treatment, the adjustments that he/she might have to do in his life style, the economical and social impact, impact on his family, etc. The Procedure done under a single skin incision, in one anatomical area, under one anesthesia, should be taken as a single Procedure.
