Results Reports. Copy Only. Participant understands that the Exchange will not deliver the official chartable report of laboratory testing results that complies with applicable reporting laws or otherwise meets the Participant’s needs. Please contact your LabCorp Account Representative to establish the LabCorp means to deliver your official chartable report of the laboratory result. initials Sharing of Data. By agreeing to these terms of use, Participant directs LabCorp to submit copies of laboratory tests results to the Exchange. Participant acknowledges that other practices and persons participating in the Exchange will have access to such results and other information. All terms related to participation in the Exchange, and for any permitted use or access of such data, are solely between Participant and the Exchange. LabCorp assumes no responsibility for how information is used once submitted into the Exchange, and Participant shall indemnify, defend and hold LabCorp harmless from and against any claims of any inappropriate use or release of information. No Supplies or Equipment. LabCorp is not providing any supplies or equipment under this agreement. Participant is responsible for arranging separately for all hardware, software, services, items, devices, or supplies necessary for Participant to connect to the Exchange. Likewise, Participant shall be responsible for all maintenance, support and service fees which are related to Participant’s system and to connection to the Exchange.
Results Reports. For each Research Program, for each Calendar Quarter during the applicable Research Term, Stoke shall provide the JDC with an update and summary of the research activities, data, results and analyses (“Results”) generated by it (or on its behalf) in conducting activities under the relevant Research Plan to date.
Results Reports. The technical agent (Consulting Firm) will provide services under this assignment when Dossiers of reasonable volume are submitted by LGED to IMED for results verification. Under this assignment, 10 (ten) tasks will be given to the Consulting Firm during the project period. LGED mandates to receive one of these services comprising cumulative information of the whole fiscal year and further update during July, August, and September each year between 2018 and 2023, which will be called Annual Result Reports, whereas other reports shall be called Intermittent Result Reports to be submitted within the completion of each task. The Annual Result Reports/Intermittent Result Reports should be strictly followed the template as provided in ANNEX-4. 10 (ten) tasks as mentioned above during the project period will be as follows: Fiscal Year No. of Task/Event for DLI verification Remark Total 10 Depending on 10 (ten) tasks/events, the technical agent (Consulting Firm) is required to submit 04 (four) Annual Result Reports depending of the availability of task(s), 10 (ten) Intermittent Result Reports and 01 (one) Completion report during the project period. The technical agent may also be required to attend Program briefings or other meetings throughout the year in order to stay up-to-date on implementation progress or unforeseen challenges. Additional services may be required and negotiated on and as per need basis. The technical agent’s (Consulting Firm) output from this assignment will involve the following reports as mentioned above: ▪ Draft Intermittent Result Report comprising preliminary findings from gross results measurements and results verification, which is to be finalized while incorporating all the comments of LGED and other Program stakeholders regarding the accuracy or completeness of the reports within two weeks of submission of the draft report; ▪ Draft Annual Results Reports comprising cumulative summary of previous reports and any update on preliminary findings from gross results measurements and results verification will be submitted on end-August of each year.
