Release of information means granting access to examine records and providing copies as requested.

This authorization includes the right for the Hospital and its affiliates, and their respective, employees, officers, directors, agents, to permit the inspection, provision, or disclosure of any documents, recommendations, reports, statements, or disclosures relating to the above. You also expressly authorize the Hospital to release this information to any third parties and its employees, officers, directors and agents upon request.
Release of information. By my signature below, I authorize Holy Cross College to release medical information regarding the above named person to any person or entity to whom Holy Cross College refers the student for medical treatment, including without limitation the Health Services at Holy Cross College.
