HEALTH OF STUDENTS. In order to be admitted into and retained in the nursing program, the individual’s physical and mental health must be suitable for performing the work required in the program and for assuring the health and safety of the public served. Nurses are considered by the public as knowledgeable in good health practices; therefore, student nurses are urged to start correcting any health problems they may have, such as poor posture, poor state of dental repair, obesity, high blood pressure, and the abuse of drugs, including nicotine, caffeine, and alcohol. Students are responsible for their own health as well as for protecting the health of their clients, classmates, and the public. Students, therefore, are not to attend clinical sites or classes on campus when ill. Students will follow program and/or course policy regarding reporting of absences. Due to the requirements at many clinical sites used by the De Anza College Nursing program, nursing students will be required to have an annual flu vaccination. If you do not get proof of vaccination from your medical provider, a form is available for your use on the de Anza nursing student website. (Please download it and take it with you to your appointment). A copy of proof of vaccination must be submitted for the nursing department records. It is the student’s responsibility to keep his/her Nursing Department file current with copies of CPR, PPD status, immunizations, post-immunization titers, physical examinations, and other essential documents. A student must process a copy of all these documents for the duration of the program and be ready to present them upon request. Students are required to provide documentation of CPR certification and QuantiFERON TB test or chest x-ray on the first day of each nursing course. No CPR certification may expire during the quarter. All documentation must be appropriate for the entire quarter, before the first day of clinical. All documentation required for the upcoming quarter must be submitted to the Resource Center Instructional Associate no later than the following deadlines: Fall Quarter First Monday of previous June (except flu shot pending availability) Winter Quarter First Monday of November Spring Quarter First Monday of March CPR certification must be ‘Healthcare Provider’ classification with a two-year certification from the American Heart Association. HEALTH OF STUDENTS. All Program participants shall provide evidence of a medical examination acceptable to Facility prior to their participation in the initial experience in the Program. Program participants shall be responsible for arranging for their medical care and/or treatment, if necessary, including transportation, in case of illness or injury while participating in the Program with Facility. Except as provided for in Exhibit A attached hereto, Facility shall not be financially responsible or otherwise responsible for said medical examination, medical care and treatment, nor shall University.

  • Admission and Enrollment of Students For a student to be accepted and enrolled into a dual credit program, the STUDENT shall:

  • Supervision of Students At least one teacher is to remain with the students after the close of any activity, practice session or game until the last student has left the premises. This rule applies whether the group is at the home school or field or is away.

  • Transportation of Students Employees will not be required to transport students.

  • Evaluation of Students Acknowledging the District’s adopted grading system, the teacher shall maintain the right and responsibility to determine grades and other evaluation of a student. No grade or evaluation shall be changed except by the teacher with the approval of the building administrator.

  • Education of Students with Disabilities 3.5.1. The DOE is the State of Hawaii's “state education agency” (SEA) and “local education agency” (LEA) for purposes of compliance with the Individuals with Disabilities Education Act (IDEA). All public schools, including charter schools, are part of and fall under the LEA. As such, the School shall comply with all applicable federal and State laws, rules, policies, procedures, and directives regarding the education of students with disabilities, including but not limited to Ch. 8-60, Hawaii Administrative Rules (HAR).

  • Health Screening The Contractor shall conduct a Health Needs Screen (HNS) for new members that enroll in the Contractor’s plan. The HNS will be used to identify the member’s physical and/or behavioral health care needs, special health care needs, as well as the need for disease management, care management and/or case management services set forth in Section 3.8. The HNS may be conducted in person, by phone, online or by mail. The Contractor shall use the standard health screening tool developed by OMPP, i.e., the Health Needs Screening Tool, but is permitted to supplement the OMPP Health Needs Screening Tool with additional questions developed by the Contractor. Any additions to the OMPP Health Needs Screening Tool shall be approved by OMPP. The HNS shall be conducted within ninety (90) calendar days of the Contractor’s receipt of a new member’s fully eligible file from the State. The Contractor is encouraged to conduct the HNS at the same time it assists the member in making a PMP selection. The Contractor shall also be required to conduct a subsequent health screening or comprehensive health assessment if a member’s health care status is determined to have changed since the original screening, such as evidence of overutilization of health care services as identified through such methods as claims review. Non-clinical staff may conduct the HNS. The results of the HNS shall be transferred to OMPP in the form and manner set forth by OMPP. As part of this contract, the Contractor shall not be required to conduct HNS for members enrolled in the Contractor’s plan prior to January 1, 2017 unless a change in the member’s health care status indicates the need to conduct a health screening. For purposes of the HNS requirement, new members are defined as members that have not been enrolled in the Contractor’s plan in the previous twelve (12) months. Data from the HNS or NOP form, current medications and self-reported medical conditions will be used to develop stratification levels for members in Hoosier Healthwise. The Contractor may use its own proprietary stratification methodology to determine which members should be referred to specific care coordination services ranging from disease management to complex case management. OMPP shall apply its own stratification methodology which may, in future years, be used to link stratification level to the per member per month capitation rate. The initial HNS shall be followed by a detailed Comprehensive Health Assessment Tool (CHAT) by a health care professional when a member is identified through the HNS as having a special health care need, as set forth in Section 4.2.4, or when there is a need to follow up on problem areas found in the initial HNS. The detailed CHAT may include, but is not limited to, discussion with the member, a review of the member’s claims history and/or contact with the member’s family or health care providers. These interactions shall be documented and shall be available for review by OMPP. The Contractor shall keep up-to-date records of all members found to have special health care needs based on the initial screening, including documentation of the follow-up detailed CHAT and contacts with the member, their family or health care providers.

  • Health Plan An appropriately licensed entity that has entered into a contract with Subcontractor, either directly or indirectly, under which Subcontractor provides certain administrative services for Health Plan pursuant to the State Contract. For purposes of this Appendix, Health Plan refers to UnitedHealthcare Insurance Company.

  • Supervision of Student Teachers Paragraph 1: Supervision of a student teacher shall be voluntary. Teachers shall normally be notified of a student teacher assigned at least two (2) weeks prior to the student teacher’s introduction to the classroom. The school system shall provide the cooperating teacher with whatever materials it deems appropriate. The cooperating teacher shall be responsible for being knowledgeable of the contents of any such materials provided.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Investigation of Accidents The Occupational Health and Safety Committees, as provided in Clause 22.2, shall be notified of each accident or injury and shall investigate and report to the Union and Employer on the nature and cause of the accident or injury. In the event of a fatality the Employer shall immediately notify the President of the nature and circumstances of the accident.
