GROUP HEALTH & LIFE INSURANCE. For those bargaining unit members employed on the date of ratification of this Agreement, group health and life insurance is carried for permanent employees and their dependents, if the employee works a minimum of fifteen (15) hours per week. For those bargaining unit members employed subsequent to the date of ratification, the minimum of fifteen (15) hours per week changes to twenty (20) hours per week. Employees who work less than twelve (12) months a year will also be covered when not on duty status, limited to 3 months per year when school is not in session. The Board shall provide at a premium cost to the eligible employee of $1080 per year payable through payroll deductions, health insurance coverage for the eligible employee and the eligible employee’s spouse and eligible dependents in accordance with the plan described in part in the attached document entitled Northwest Arctic Borough School District Health Care Plan, Group No. 12431, Summary Plan Description, including the Health Care Review Service. The premium payment shall be retroactive to July 1, 2017, for those bargaining unit members employed on March 27, 2018. The District has negotiated a steerage preferred provider/network plan for hospitalization (in-patient and out- patient) and service providers The penalty co-payment shall be 40 percent for Out of Network hospitalization, thereby reducing the District’s co-payment from 90% to 50%. The penalty co-payment shall be 20% for Out of Network service providers, thereby reducing the District’s co-payment from 90% to 70%. An eligible employee who chooses not to pay that yearly premium cost shall waive the Board provided health insurance coverage, including such coverage as a spouse or dependent. Said premiums shall be eligible for the District’s Section 125 plan. Coverage shall begin on the first day of the month coinciding with, or following, the eligible employee’s first day of work for the District. Coverage shall terminate on the last day of the month coinciding with, or following, the eligible employee’s last day of work for the District. GROUP HEALTH & LIFE INSURANCE. A. The Board will offer health care and life insurance as listed in Section F below to each full-time employee and his/her family if applicable. The full-time employee electing such coverage shall pay, by payroll deduction, 12% of the employee premium for those employees who elect Option A, composed of medical plan 7A and RX Plan 4. (A description of the coverages provided in Option A is set forth below.) Employees who elect to continue with the current health insurance program (Option B) shall pay 15% of the employee premium cost. The Option A and B plans are more completely described below. Each year of the agreement during the open enrollment period, employees may elect the plans associated with options A (12% employee contribution) or B (15% employee contribution) for that year. Coverage shall be effective the first day worked, and the employee shall have the option of selecting single or family coverage.

  • Group Life Insurance The Hospital shall contribute one hundred percent (100%) toward the monthly premium of HOOGLIP or other equivalent group life insurance plan in effect for eligible full-time employees in the active employ of the Hospital on the eligibility conditions set out in the existing Agreements.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • Life Insurance No portion of your IRA may be invested in life insurance contracts.

  • Group Health Insurance Immediately following retirement, the teacher shall have the option of remaining in the Corporation’s current group health insurance plan if all of the following conditions are met as of the date of retirement and thereafter:

  • Group Term Life Insurance The School District will pay the full premium for each $1,000 of coverage for group term life insurance for all full-time teachers who qualify for and enroll in the existing group term life insurance plan of the School District. Full-time teachers who qualify and enroll will be insured with $50,000 of group term life insurance.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.

  • Hospitals of Ontario Voluntary Life Insurance Plan The Hospital also agrees to make the Hospitals of Ontario Voluntary Life Insurance Plan (HOOVLIP) available to the nurses subject to the provisions of HOOVLIP at no cost to the Hospital.

  • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.

  • Term Life Insurance The Employer will maintain and make available to full-time and part-time employees, the current term life insurance plan as set forth in the document "Summary of Health Benefits, Maryland State Employees."
