DISTRICT BENEFITS. Tuolumne Utilities District offers a comprehensive package of group insurance and other benefit programs to its employees. Complete and official details of the group insurance are contained in plan documents. The plans are briefly highlighted below. Refer to official plan documents for important details not covered here. Benefits are available to full-time regular employees (scheduled to work 30 hours or more per week). It is recognized that such plans may change as to scope of benefits offered and deductibles charged by the insurance provider, and that such changes are beyond the ability of the Employer to control. The employer has the authority to review other insurance plans, if those plans will provide equal or better coverage. Implementation of any new medical plan covering employees and retirees and their dependents must receive union approval.

DISTRICT BENEFITS. Each licensed employee may participate in a district sponsored IRS Section 125 cafeteria style fringe benefit salary reduction plan. The benefits shall include, but not be limited to, term life insurance, health insurance, cancer insurance, salary protection, dependent care reimbursement, un-reimbursed medical and stand alone dental coverage. The option(s) selected by the licensed employee shall be made known to the central office in writing. The health insurance option shall include a payment from the district of $400.00 per month toward the premium amount of any district sponsored health insurance plan. Only employees enrolled in one of the district health insurance plans are eligible for this payment. No cash option is provided as part of this payment. In the event that the spouse of the employee is also an employee of the district, and if only one of the above employees is enrolled in a district sponsored family health insurance plan, and if the spouse is covered under the above plan, the district payment shall be $800.00 per month toward the premium of the health insurance. No changes are allowed after the plan year sign-up date unless they are in accordance with Internal Revenue Service guidelines. Any health insurance premium refunds shall be returned to both Xxxxxx USD 345 and individual policy-holders in proportion to the amount of money contributed by each toward the annual premium cost of each policy.

  • Unemployment Benefits The Company will not oppose the Executive’s claim for unemployment insurance benefits.

  • General Benefits During the Term of Employment, the Executive shall be entitled to participate in such employee pension and welfare benefit plans and programs of the Company as are made available to the Company's senior-level executives or to its employees generally, as such plans or programs may be in effect from time to time, including, without limitation, health, medical, dental, long-term disability, travel accident and life insurance plans.

  • Covered Benefits and Services The Contractor shall provide to its Hoosier Healthwise members, at a minimum, all benefits and services deemed “medically reasonable and necessary” and covered by the IHCP, and included in the Indiana Administrative Code and under the Contract with the State. A covered service is considered medically necessary if it meets the definition as set forth in 405 IAC 5-2-17. The Contractor shall deliver covered services sufficient in amount, duration or scope to reasonably expect that provision of such services would achieve the purpose of the furnished services. Costs for these services are the basis of the Contractor’s capitation rate and are, therefore, the responsibility of the Contractor. Coverage may not be arbitrarily denied or reduced and is subject to certain limitations in accordance with CFR 438.210(a)(4), which specifies when Contractors may place appropriate limits on services:  On the basis of criteria applied under the State plan, such as medical necessity; or  For the purpose of utilization control, provided the services furnished are sufficient in amount, duration or scope to reasonably be expected to achieve the purpose for which the services are furnished.

  • ELHT Benefits The Parties agree that since all active eligible employees have now transitioned to the OSSTF ELHT all references to existing life, health and dental benefits plans in the applicable local collective agreement for active eligible employees shall be removed from that local agreement. Post Participation Date, the following shall apply:

  • Public Benefits This Agreement provides assurances that the Public Benefits identified below will be achieved and developed in accordance with the Applicable Rules and Project Approvals and with the terms of this Agreement and subject to the City’s Reserved Powers.   • Medical Benefits The Company shall reimburse the Employee for the cost of the Employee's group health, vision and dental plan coverage in effect until the end of the Termination Period. The Employee may use this payment, as well as any other payment made under this Section 6, for such continuation coverage or for any other purpose. To the extent the Employee pays the cost of such coverage, and the cost of such coverage is not deductible as a medical expense by the Employee, the Company shall "gross-up" the amount of such reimbursement for all taxes payable by the Employee on the amount of such reimbursement and the amount of such gross-up.

  • Health Benefits The method for determining the Employer bi-weekly contributions to the cost of employee health insurance programs under the Federal Employees Health Benefits Program (FEHBP) will be as follows:

  • Death Benefits Upon the Executive's death during the Contract Period, his estate shall not be entitled to any further benefits under this Agreement.
