Collection Sites. Collection services will be provided at the OMEGA Clinic (the City’s physician group). For services when OMEGA is not open, collection services will be provided at the Evanston Hospital Emergency Room.
Collection Sites. A facility provided by CONTRACTOR, and approved by the County Program Manager, where COUNTY clients present themselves for the purpose of providing a specimen of their urine to be analyzed for the presence of drugs. Washington State Liquor and Cannabis Board (WSLCB) Reasonable Suspicion Training Guidelines Contractor for Drug & Alcohol Testing Services contract #00818 shall abide by Washington State Liquor and Cannabis Board (WSLCB) requirements for Reasonable Suspicion Training. Contractor will provide the WSLCB with Reasonable Suspicion Training on an as needed basis. The Rate for Reasonable Suspicion Training shall be based on the hourly rate provided within the contract price sheet.

Collection Sites. An example collection site list is shown as Attachment1. The Contractor shall provide an updated collection site list should any information in Attachment 1 change during the term of the Contract. At a minimum, the Contractor shall provide at least one collection site for urine specimen and breath alcohol testing in all 95 counties in Tennessee to the extent possible. The site must be open and available during normal business hours; i.e. 7:00 a.m. to 4:00 p.m., 8:00 a.m. to 5:00 p.m., 9:00 a.m. to 6:00 p.m., Monday through Friday in the time zone of the location. If a collection site cannot be located in a County, the nearest possible site in another County should be used. Site and hours of operation changes are subject to prior written approval of the State. The Contractor shall provide testing services 24 hours per day, 7 days per week for unscheduled post-accident and reasonable suspicion testing. The Contractor's after- hours contact information (i.e., pager#, cell phone#, etc.) should be provided. Should this contact information change during the term of the contract, the State must be notified in writing within one day.
Collection Sites. Collection services will be provided at a clinical laboratory or hospital facility, which is certified by the State of Illinois to perform drug and alcohol testing. The District will endeavor to have nonscheduled tests conducted at a facility other than an emergency room at which the District regularly transports patients.
Collection Sites. The designated collection site shall comply with all requirements specified in 49 CFR Part 40, Subparts D and K. Procedures for collecting urine specimens shall allow individual privacy unless there is reason to believe a particular individual may alter or substitute the specimen to be provided. No unauthorized personnel shall be permitted in any part of the designated collection site where specimens are collected or stored. Contractor shall provide for the collection site to be secure at all times. The collection site shall be dedicated to drug and alcohol testing and the portion of the facility used for testing shall be secure at all times.
