SERVICES TO BE FURNISHED BY CONSULTANT. In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate regulations and requirements of the City of Westfield, the Indiana Department of Transportation, and Federal Highway Administration. The CONSULTANT shall be responsible for performing the following activities: Project Description WSP USA Inc (the CONSULTANT), is the prime design Consultant to the City of Westfield (the CLIENT) for the preliminary engineering for reconstruction of Little Eagle Creek Avenue from 000xx Xxxxxx to Shelborne Road; total reconstruction length of approximately 1.58 miles long. The project is a federally funded project in the City of Westfield, Xxxxxxxx County, Indiana. The CONSULTANT shall be responsible for the management, coordination, design and development of plans necessary to produce construction contract documents so that others may construct the improvements for the roadway reconstruction on Little Eagle Creek Avenue as identified. The design shall be developed in accordance with the following documents in effect at the time the plans or reports are submitted: American Association of State Highway and Transportation Officials' "A Policy on Geometric Design of Highways and Streets"; Indiana Department of Transportation's Standard Specifications; Manual on Uniform Traffic Control Devices; Road and Traffic memoranda and INDOT Design Manuals (IDM), except as modified by supplemental specifications and special provisions; City of Westfield Construction Standards and Specifications (Rev March 2017). The work shall be submitted electronically in Adobe Acrobat (.pdf) format via INDOT’s electronic plan submittal process (ERMS) to INDOT, and via e-mail, ftp site, or similar to the City of Westfield. The CONSULTANT shall coordinate its designs with guidance from the CLIENT on scope, schedule, and design-related technical issues, subject to approval by INDOT or Federal Highway Administration (FHWA). In fulfillment of this Contract, the CONSULTANT shall comply with the requirements of the appropriate regulations and requirements of the City of Westfield and the State of Indiana. The project area includes North Street from East Street westward to the ending at the alley. The essence of the project is to prepare updated survey data, construction documents and public information for the improvements in the project area. Toward this end, the CONSULTANT shall be responsible for performing the following activities: Topographic Survey of Project Areas, as follows: • Along North Street from East Street westward to termination at alley. • From 5’ beyond north R/W to 5’ beyond south R/W • Determine casting elevations, invert elevations, pipe materials and pipe sizes • Assess existing conditions w/ photos Construction Documents for Full Reconstruction of North Street per Westfield Construction Standards (INDOT IDM as needed) • One travel lane in each direction • New curb and gutter and drainage • Provide new ADA-compliant curb ramps • Provide new driveways where directed by the City • ADA-compliant pavement markings and signage o Salvage existing signs where prudent o Adjust existing castings to grade Additional Assumptions Affecting Scope of Services: • Provide Utility Coordination in accordance with 105 IAC 13 • Prepare SWPPP and obtain Rule 5 permits • Provide Engineer’s Final Cost Estimate and Quantity Calculations • Attend and provide Support for Public Information Meetings • Prepare Bid documents including final plans and unique special provisions as needed. • Provide final CAD to City of Westfield GIS for recording • Post‐Bidding Services, as necessary • No R/W acquisition is anticipated Items to be provided by the City of Westfield: • Coordination, Presentation, and meeting space for the Public Information Meetings • Coordination with future projects such as SR 32 Reconstruction in downtown Westfield Project letting and advertising • Contract Book preparation Construction Inspection • New construction As‐built data • Support on Utility Coordination by facilitating meetings as needed APPENDIX "B"

SERVICES TO BE FURNISHED BY CONSULTANT. Consultant shall perform additional topographical survey along the revised Xxxxx Xxxxxxxx Xxxxx 0X route, as depicted on the figure included in Attachment 1. Consultant shall develop existing property and right-of-way linework for parcels along the revised route. Consultant shall coordinate with A&F Engineering to determine the approximate location of the Florida Road realignment. Consultant shall perform additional engineering and plan development services for the revised route, and hold a site meeting with LPA to review the revised design. Consultant shall perform additional utility coordination services for the revised route. Consultant shall coordinate the revised route with the environmental consultant, Lochmueller Group. Consultant shall perform right-of-way engineering for the two additional parcels along the new route. APPENDIX "B" INFORMATION AND SERVICES TO BE FURNISHED BY THE LPA: Not applicable to this supplemental agreement. APPENDIX "C" SCHEDULE: No work under this Contract shall be performed by the CONSULTANT until the CONSULTANT receives a written notice to proceed from the LPA. All work by the CONSULTANT under this Contract shall be completed and delivered to the LPA for review and approval within the approximate time periods shown in the following submission schedule:
