MEDICAL, DENTAL & LIFE INSURANCE. 12.1 Virtua shall continue to offer to full and part time employees who work at least thirty-five (35) hours per pay period, medical, dental, prescription and life insurance benefits under the same terms and conditions (including the same costs of coverage, premium contributions, co-pays and deductibles, and medical waiver credits) as those benefits are provided to similarly situated non-management, non-bargaining unit employees of Virtua. Virtua shall have the discretion to change, alter and/or amend these benefits (including changing cost of coverage, premium contributions, co-pays and deductibles, plan design, insurance carriers and/or becoming fully or partially self-insured, and making such changes as may be required by applicable law), so long as such changes, alterations and/or amendments apply equally to the employees covered by this Agreement and similarly situated non-management, non-bargaining unit employees of Virtua. Virtua, in its sole discretion, may offer more than one option for benefits (for example, a PPO or HMO), and the parties further acknowledge that Virtua, in its discretion, may add or delete a particular option so long as such additions and/or deletions apply equally to employees covered by this Agreement and to similarly situated non-management, non- bargaining unit employees of Virtua. Virtua shall give the Union sixty (60) days’ written notice of any change in employee contribution levels or plan design and provide supporting documentation, including the cost to the Employer.
MEDICAL, DENTAL & LIFE INSURANCE. 28.1 Medical Insurance: Effective January 1, 1991, the City will only pay 100% of the premium for the applicable (single, two party, family) Kaiser rate. Plans that are less expensive than or equal to the Kaiser plan shall be fully paid by the City. The cost difference between the Kaiser Plan and more expensive plans will be borne by the employee. Dependent status will be available until the dependent reaches the age of twenty-four (24) providing the dependent(s) meet the definition of “dependency” in the Internal Revenue Code. Effective January 1, 2009, the amount the City contributes each calendar year toward the payment of health insurance premiums will increase by the lesser of twenty percent (20%) (single, two party, family) or the amount of the Kaiser HMO premium amount (single, two party, family) in effect on that date. The amount the City contributes each calendar year uses the premium rate the previous calendar year as the basis for the calculation and there shall be no year-to-year carryover. If an employee chooses to complete and submit an Affidavit of Domestic Partnership and sign up for medical benefits for his or her domestic partner, the employee shall be subject to federal and state income tax withholding.

MEDICAL, DENTAL & LIFE INSURANCE. King County presently participates in group medical, dental and life insurance programs. The County agrees to maintain the level of benefits in these plans during the term of this Agreement, provided that the Union and County agree to incorporate changes to employee insurance benefits which the County may implement as a result of the agreement of the Joint Labor-Management Insurance Committee. Coverage eligibility will be governed by the 2005 King County Personnel Guidelines.
MEDICAL, DENTAL & LIFE INSURANCE. The School District agrees to contribute the premium as per the terms of this Agreement for medical-hospitalization, dental, and life insurance for any director who elects early retirement/resignation/severance under this Agreement. Such District contribution shall be continued up to a maximum total of ten years. The School District may utilize a Retiree Health Reimbursement Arrangement of the District’s choosing to facilitate this provision.
MEDICAL, DENTAL & LIFE INSURANCE. The City shall provide every eligible employee (defined as an employee receiving benefits) with the option of selecting medical and/or dental and/or vision insurance for the employee only or for the employee and all eligible family members. If eligible family members are enrolled, they must be enrolled in the same coverage as elected by the employee.

  • Supplemental Life Insurance In addition to the life insurance benefits provided by this agreement, employees may subscribe voluntarily and at their own expense for supplemental life insurance. Employees may subscribe for an amount not to exceed five hundred thousand dollars ($500,000), of which one hundred thousand ($100,000) is a guaranteed issue, provided the election is made within the required enrollment periods.

  • Term Life Insurance The Employer will maintain and make available to full-time and part-time employees, the current term life insurance plan as set forth in the document "Summary of Health Benefits, Maryland State Employees."

  • Life Insurance No portion of your IRA may be invested in life insurance contracts.

  • Group Term Life Insurance The School District will pay the full premium for each $1,000 of coverage for group term life insurance. The amount of life insurance provided will be $20,000, subject to the conditions of the carrier.

  • Group Life Insurance The Hospital shall contribute one hundred percent (100%) toward the monthly premium of HOOGLIP or other equivalent group life insurance plan in effect for eligible full-time employees in the active employ of the Hospital on the eligibility conditions set out in the existing Agreements.

  • Dependent Life Insurance In the event of the death of your spouse or dependent child from any cause whatsoever, while you and your dependents are insured under the plan, the insurance company will pay you $10,000 in respect of your spouse and $5,000 in respect of each insured dependent child. This applies to those employees with family health coverage only.

  • Retiree Life Insurance Employees who retire under the Monroe County Employees' Retirement System shall be eligible for $4,000.00 term life insurance. All employees hired by the Employer on or after October 1, 2007 shall not be eligible for Retiree Life Insurance.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • Medical, Dental and Vision Insurance The Employer provides health care coverage for medical, dental and vision for the employee, spouse and children (one-person, two-person and family) subject to the provisions of this Article. Currently the plan options are:

  • Key Man Life Insurance The Company may apply for and obtain and maintain a key man life insurance policy in the name of Executive together with other executives of the Company in an amount deemed sufficient by the Board, the beneficiary of which shall be the Company. Executive shall submit to physical examinations and answer reasonable questions in connection with the application and, if obtained, the maintenance of, as may be required, such insurance policy.
