Medical. There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.
Medical. 12.1 You acknowledge that it is an essential requirement of this Agreement that the NZOC is kept fully informed if you are suffering any physical or psychological injury, illness, condition or impairment that might prevent you from carrying out your Support Staff role at the Games to the highest possible standard. Medical. Commencing 7/1/1999, the District will make available to each member the cost of the member’s premium for the member’s HMO plan. If for some reason a member does not wish to enroll in a District- sponsored medical plan, the member may elect to receive $200 for the year in lieu of medical insurance to be applied to other insurance benefits or the Section 125 IRS plan, as approved by the District.
Medical. Upon the request of a permanent employee who has exhausted all accrued sick leave, a leave of absence without pay shall be granted by the Appointing Authority for up to one (1) year because of sickness or injury to the employee. At the request of the employee, this leave may be extended at the discretion of the Appointing Authority. An employee requesting a medical leave of absence shall be required to furnish evidence of disability to the Appointing Authority. When the Appointing Authority has evidence that an employee's absence from duty is unnecessary or if the employee fails to undergo an evaluation or furnish such reports as are required by the Appointing Authority, the Appointing Authority shall have the right to require the employee to return to work on a specified date.
Medical. Current health insurance coverage is required for every student. Students are eligible to enroll in a University health policy if they are not covered. Enrollment forms are available through the Business Office. Students must provide proof of health insurance prior to being assigned housing. Students will be automatically billed for health insurance unless the online waiver is completed.
Medical. Each employee shall pay the following portion of the premium or premium equivalent for the above medical plans for the coverage of the employee and their eligible dependents. For the purposes of the benefit plans set forth in this Article, “eligible dependent” shall be a spouse or child who meets the criteria set forth in the insurance carrier’s plan description. Any employee who receives benefits for dependents who do not meet the requirements of Section 152 of the Internal Revenue Code shall be solely responsible for any resulting taxes and related charges, and shall hold the City harmless from any costs in connection with the provision of such benefits. HDHP: Effective September 1, 2018, 20% of the premium equivalent. HDHP: Effective September 1, 2019, 21% of the premium equivalent. Effective September 1, 2020, 21.5% of the premium equivalent. The employee’s premium contribution shall be increased over the previous year’s employee’s premium contribution by .5%.
Medical. If Member is medically unable to use the CRUNCH facilities, Member may request a freeze of his or her membership in monthly increments for a minimum of 1 month and a maximum of 6 months per year. Members must provide a doctor’s note at the time of requesting a freeze. CRUNCH reserves the right to verify the note with the doctor and Member agrees to waive any Doctor/Patient privilege and execute any release required by such doctor solely with regards to the medical reasons stated in the doctor’s note for the medical freeze.(b) Non-medical, if a Member desires to freeze his or her account for a non-medical reason, Member should consult with Member’s CRUNCH facility. CRUNCH reserves the right to adjust this freeze policy from time to time at is sole discretion. All requests must be received at least ten (10) days prior to Member’s next billing date period. Retroactive freezes will not be accepted. Certain fees may apply to any freeze of a Member’s membership. Member affirms, acknowledges and attests that Member’s mailing address, telephone number, cellular telephone number and e-mail address provided on the face of this agreement are accurate and were provided by Member voluntarily. Subject to applicable law, Member agrees that Crunch and ABC Global Services, including its agents and affiliates, may contact Member at any mailing address, telephone number, cellular telephone number or e-mail address set forth on the face of this agreement, or subsequently provided by Member to Crunch and/or ABC Global Services. E-SIGN Consent. Certain laws and regulations may require Crunch and/or ABC Global Services, to provide Member with written notices and disclosures on paper. With Member’s consent, this information may be provided to Member electronically. Member’s consent hereto shall apply to each and every disclosure, notice, agreement, statement, term and condition, and any other information (collectively, the “Documents”) that Crunch and/or ABC Global Services, may provide Member. Member’s consent to receive the Documents electronically shall continue until expressly withdrawn by Member. Notwithstanding, Member may request a paper copy of all electronic Documents by contacting Crunch and/or ABC Global Services, and requesting a paper copy. Member may withdraw its consent at any time by notifying the customer service department of Crunch and/or ABC Global Services. Medical. A. The District agrees to continue medical coverage at the level provided in this MOU. Employees will pay 15% of the cost of the premium for medical coverage and the District will pay 85% of the cost of the premium for all employees and their dependents, including registered domestic partners. During the life of the MOU, any increases or decreases in premium rates will also increase or decrease the total amount paid by the established cost–sharing. Employee paid medical premiums may be paid on a pre–tax basis in accordance with the IRS Section 125 Plan.
