HOSPITAL MEDICAL PLAN. The Extended Health Benefit Plan is integrated with the Medical Services Plan of British Columbia. Employees are required to complete the necessary enrolment forms within thirty-one (31) days of first becoming eligible. Failure to apply within the thirty-one (31) days may cause a penalty of two (2) months additional waiting period for coverage in accordance with the provincial rules for eligibility. Please note that transfers from other Medical Services Plan coverage within thirty (30) days of cancellation is excluded from the employment penalty clause.

MEDICAL SERVICES PLAN OF BRITISH COLUMBIA Eligibility Effective December 1, 2000 new employees will be entitled to MSP-BC after six (6) months service and contingent on maintaining a weekly average of twenty (20) hours or more consistent with the policy contained in the White Spot Master Agreement. Effective December 1, 2000 employees of record as of November 30, 2000 will be entitled to MSP-BC on the first day of the month following six (6) months service. Effective June 1, 2001 employees of record as of November 30, 2000 will be entitled to MSP-BC after six (6) months service and contingent on maintaining a weekly average of ten (10) hours or more consistent with the policy contained in the White Spot Master Agreement.

HOSPITAL MEDICAL PLAN. The Company agrees to maintain the current or a substantially equivalent group health insurance plan(s) for regular full-time employees. The Company shall pay an amount equal to eighty percent (80%) of the lowest cost plan offered, single or family, for employees electing coverage under a Company sponsored plan. The employee shall pay the remaining premium by payroll deduction. An employee on an unpaid leave of absence or layoff may maintain his coverage during the leave or layoff in accordance with the plan.

  • Medical Plan ‌ Eligible employees and dependants shall be covered by the British Columbia Medical Services Plan or carrier approved by the British Columbia Medical Services Commission. The Employer shall pay one hundred percent (100%) of the premium. An eligible employee who wishes to have coverage for other than dependants may do so provided the Medical Plan is agreeable and the extra premium is paid by the employee through payroll deduction. Membership shall be a condition of employment for eligible employees who shall be enrolled for coverage following the completion of three (3) months’ employment or upon the initial date of employment for those employees with portable service as outlined in Article 14.12.

  • Health Plan An appropriately licensed entity that has entered into a contract with Subcontractor, either directly or indirectly, under which Subcontractor provides certain administrative services for Health Plan pursuant to the State Contract. For purposes of this Appendix, Health Plan refers to UnitedHealthcare Insurance Company.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Medical Plans The Employer will maintain the current health (including vision) and dental insurance programs and practices. The Employer shall contribute 80% of the premium charge for PPO plans, 83% of premium for the POS plan, 85% of premium for the HMO plan, 80% for the prescription drug plan and 50% for the dental plan. There shall be no change in the State’s premium subsidy for health benefits plans in Fiscal Year 2012.

  • Medical There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.

  • Vision The University shall make available vision insurance to the staff members covered by this agreement to the same extent and in the same manner as is available to other University staff members, such as Faculty and the Executive, Administrative and Professional Staff members. It is the University's goal to have the same vision insurance plan(s) offered uniformly to all University staff member groups and staff members.

  • Medical Care The Parents must comply with the School Welfare Officer's recommendations which may include a reasonable decision to release the Pupil home or to his / her education guardian when s/he is unwell.

  • MEDICALLY FRAGILE STUDENTS 1. If a teacher will be providing instructional or other services to a medically fragile student, the teacher or another adult who will be present when the instruction or other services are being provided will be advised of the steps to be taken in the event an emergency arises relating to the student's medical condition.

  • Dental Care Plan The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred in respect of the coverages summarized in Appendix "1". The Plan will not duplicate benefits provided now or which may be provided in the future by any government program.

  • Wellness A. To support the statewide goal for a healthy and productive workforce, employees are encouraged to participate in a Well-Being Assessment survey. Employees will be granted work time and may use a state computer to complete the survey.
