Employee Wellness Program. The Calumet County Employee Wellness Program's primary purpose is to promote healthy lifestyles for employees and covered spouses. The following are the criteria for the Employee Wellness Program which allows employees to receive a reduced premium contribution:
Employee Wellness Program. ESP members shall be eligible to participate in the College’s Employee Wellness Program.

EMPLOYEE Wellness Program (EWP): The Superintendent and the Union recognize the necessity for identifying, developing, promoting, and promulgating sound physical and mental health and developing prevention and correction programs for District employees. As a result of this awareness, the Superintendent has established the EWP.
15.5.2. The basic elements of the program may include, but are not limited to: mandatory and self- referral of employees to community resources for counsel or treatment, developing programs for stress reduction, and/or developing procedures for identification and referral of employees in need of assistance.

Employee Wellness Program. 26.1 Each member may voluntarily submit to a wellness physical examination at a location selected by the Town and at the Town’s expense. Each Party recognizes that maintaining the health and fitness of the member can be a matter of life or death. Results of said wellness physical examination are confidential between the Wellness Center and the employee only and will not, under any circumstances, be furnished to the Town.
26.2 This wellness examination shall be yearly for all employees over the age of forty and every two years for employees under the age of forty.
Employee Wellness Program. The Employer may offer an Employee Wellness Program, in accordance with applicable laws and regulations. Employees who elect to participate in the Employee Wellness Program in 2022 and complete the three (3) requirements for participation in the Wellness Program will receive a yearly rebate totaling $250 for single coverage and $600 for family coverage, which will be broken down into equal reductions in the premium amount deducted from the employee’s paycheck each pay period in 2022. Employees who elect to participate in the Employee Wellness Program in 2022, complete the three (3) requirements, and complete nine (9) additional wellness activities as designated by the Employer will receive an additional yearly rebate beginning on April 1, 2022, totaling $100 if the Employee completes the requirements and $250 if the Employee and covered spouse complete the requirements. The same incentives for participation in the Employee Wellness Program will be offered in 2023 and 2024. The Employer shall hold quarterly meetings to update the union and employees on the County’s insurance plans and Wellness Program. The Union President may designate up to three (3) bargaining unit members to attend the meetings. The meetings shall last no more than one (1) hour and the bargaining unit members shall be compensated at their regular hourly rate for time spent at the meetings.
