Annual Performance Assessment. 15. With regard to Xx. Xxxxxx’x employment as the Superintendent, the Board shall evaluate in writing the performance of Xx. Xxxxxx at least once a year during the Term of this Employment Contract, on or before June 30th of each year, unless the parties mutually agree in writing on another date for the annual performance assessment. The annual performance assessment shall be conducted in an executive session limited to members of the Board and Xx.
Annual Performance Assessment. General Criteria and Procedures The College is committed to excellence in teaching, professional activities, scholarship and service. It seeks to provide opportunities for Faculty to engage in research for the expansion and application of knowledge and for a greater understanding of the world in which we live. It encourages Faculty to engage in professionally related service that contributes to the greater good of the institution, the professions, the community and the nation. The standards and processes by which Faculty are assessed play a major role in ensuring the College’s ability to achieve this mission. The College seeks a Faculty assessment and evaluation system that encourages the kinds of performance that support its mission and promote the professional development of Faculty. Faculty shall undergo an annual assessment that is fair, equitable in its outcomes, simple in its implementation and effective in promoting the kinds of performance that will enhance the institution’s mission.
Annual Performance Assessment. Xx. Xxxxxx shall be evaluated annually by the Board on or before May 1 of each year of this Agreement. The performance of Xx. Xxxxxx shall be assessed in part against the objective performance standards that have been mutually agreed upon by the Board and Xx. Xxxxxx subject to the requirements of Act 82 of 2012. As required by law, the Board shall post annually the mutually agreed upon objective performance standards on the District website and shall also annually post the date of Xx. Xxxxxx’x annual performance assessment and whether or not Xx. Xxxxxx met the agreed upon objective performance standards. No other information regarding Xx. Xxxxxx’x performance assessment shall be posted on the District website or in any other manner disclosed by the District unless expressly required to do so by state or federal law. The Board and Xx. Xxxxxx hereby mutually agree to establish objective performance standards on or before June 15, 2023, which shall be reviewed and updated as necessary on or before June 15 of each year of this Agreement unless another date is mutually agreed upon by the Board and Xx. Xxxxxx.

HHA and its personnel shall at all times during the term of this Agreement continue to meet all the requirements detailed in this Agreement, including providing reputable, high quality home health services, in accordance with the accepted medical standards, regulations and professional guidelines. In furtherance thereof, the HHA will participate at least once annually and otherwise as requested by UCI, in an eligibility and performance assessment with UCI. During each such assessment, UCI will discuss with the HHA its performance against quality and process metrics and whether HHA is working toward the baseline target for each metric.

SNF and its personnel shall at all times during the term of this Agreement continue to meet all the requirements detailed in this Agreement, including providing reputable, high quality skilled nursing services, in accordance with the accepted medical standards, regulations and professional guidelines. In furtherance thereof, the SNF will participate at least once annually and otherwise as requested by UCI, in an eligibility and performance assessment with UCI. During each such assessment, UCI will discuss with the SNF its performance against quality and process metrics and whether SNF is working toward the baseline target for each metric. SNF shall ensure that the percentage of undocumented or unfunded patients accepted shall be at a minimum of ten (10) percent of the total patients accepted by SNF as part of the referral from UCI.
Annual Performance Assessment. After the first year of this Agreement, the FAME Board shall evaluate, in writing, the performance of Xx. Xxxxx at least once a year during the term of this Agreement, no later thanMay 30 of each year, using a mutually agreed upon method as the basis for said evaluation, provided that any assessment system selected shall require the FAME Board to speak in one voice by voting as an entire board rather than “averaging” the feedback of each member regarding each aspect of the evaluation. In the event the FAME Board’s consensus determines that the performance of Xx. Xxxxx is unsatisfactory in any respect, it shall describe in writing, and in reasonable detail, the specific instances of unsatisfactory performance. A copy of the written evaluation shall be delivered to Xx. Xxxxx. Xx. Xxxxx shall have the right to make a written response to the evaluation. The FAME Board’s evaluations and Xx. Xxxxx’x responses shall be totally private and in no manner become public knowledge. The parties shall have the right to mutually waive a formal performance assessment in any year of the Agreement provided; however, that Xx. Xxxxx shall not be subject to discipline or discharge on the basis of neglect of duty in any year when a formal performance assessment is not completed.
