Schedule of Insurance. Certificate means a certificate issued by Us, and, if more than one, then the latest in time. The Schedule of Insurance Certificate contains details of the Policyholder, Insured Persons and the Benefits applicable under the Policy.

Dental Plan Eight units per year per person of periodontal scaling will be added to the Plan.

Schedule of Insurance. 1. WORKERS COMPENSATION - A policy covering the obligations of the Design-Builder in accordance with the Workers Compensation Law and the Disability Benefits Law covering all operations under this Contract whether performed by the Design-Builder or its Contractors.
