Certificate of Insurance. Contractor must provide a Certificate of Insurance form to the City of Sparks to evidence the insurance policies and coverage required of Contractor.

Certificate of Insurance. The Xxxxx 25 Certificate of Insurance form or a form substantially similar must be submitted to the City to evidence the insurance policies and coverages required of Contractor.

The certificate must specify an additional insured endorsement, and Contractor shall attach a copy of the endorsement to the certificate.
