MINOR CHILDREN. With respect to any and all minor children (under the age of 18 years – “Minor”) brought onto the Property by any Guest, it will be the sole and exclusive duty and obligation of such Guest to care for and protect each of such Minors at all times while traveling to and from the Property and on the Property. The Guest shall take all measures to ensure that such Minor(s) are properly licensed by all state and federal game management agencies for the appropriate age of such Minor and will ensure that all such Minor(s) abide by all state and federal game laws and Rules of the Property as instituted by Xxxxxx Ranch and Hat Creek. Guest warrants and represents that such Minor has successfully completed all required hunter safety course(s). The Guest will further ensure that such Minor will be properly supervised with regard to the use of firearms while on the Property. Such Guest will be responsible for any injury to, or damage to the Property, by any such Minor. To the full extent permitted by law, the Guest hereby agrees that each Minor will be subject to all of the terms, conditions and provisions of this Agreement and hereby give all legal and necessary consent related thereto.
MINOR CHILDREN. Charges for minor children will be billed to the parent with whom the child resides, or to appropriate insurance if all required information is provided. We will not bill or contact a non-custodial parent on behalf of the custodial parent.
MINOR CHILDREN. User agrees that in the event it has any minor children under its supervision while on the Premises, it will ensure that criminal background checks have been conducted for all personnel who will be interacting or may interact with the minor children. User further agrees that it will not allow anyone convicted of a sexual offense to be employed or volunteer in any capacity. User hereby indemnifies and holds FIU harmless from and against any and all claims, losses or expenses that may arise in connection with the provisions contained herein.
MINOR CHILDREN. In the event a minor is traveling with only one parent or legal guardian, notarized documentation signed by the absent parent or legal guardian must be submitted to CLA, releasing the company from any responsibility at least 24 hours prior to departure.
MINOR CHILDREN. In the event that I have minor children who will accompany me on my assignment, I take full responsibility for their supervision and conduct at all times, and I, acting both on my own behalf and on their behalf as their parent and legal guardian, do hereby assume all risks of death, illness, or injury that they may suffer as a result of said appointment, from those causes described above.
MINOR CHILDREN. The adult accompanying the minor is responsible for payment of services. Minors must always be accompanied by an adult. Helpful Q&A Q: Will you accept assignment of my benefits? A: Yes, as long as the benefit check can be sent our office. Q: How much will my portion be if I have dental treatment provided? A: 30% - 60% of the treatment fee is required at the time of service. The amount varies according to your insurance plan and the procedure provided. We provide estimates to all patients’ prior to service.
