Life and AD&D Insurance. 17.2 The Group Life and Accidental Death and Dismemberment Insurance shall provide a benefit of two (2) times annual salary to a maximum benefit of $250,000. Entitlements under the Group Life and AD & D insurance plans for employees receiving long-term disability insurance are based on the members’ salary at the commencement of disability.
Life and AD&D Insurance. Eligible employees may opt to purchase a group term life insurance policy in the amount of $10,000 with a carrier selected by the City. The premium cost of the policy shall be borne fully by the employee. The City will provide up to $50,000 Accidental Death and Dismemberment (AD&D) insurance subject to the terms and conditions of the City’s policy. This benefit will be provided at no cost to the employees.
Life and AD&D Insurance. The City shall provide, at no cost to the employee, term life insurance in an amount equal to his annual base pay rounded to the nearest thousand dollars. Employees also have the option of purchasing additional life insurance in accordance with the options available for which they are eligible. The available options are explained in the City’s Benefits Line which is updated annually prior to open enrollment for benefit selections. It is agreed by the parties that the rates paid by the employee for supplemental term life insurance will be established by the City’s vendor and may be changed in the event the vendor implements a rate change.
Life and AD&D Insurance. The Employer shall provide $15,000 in Life Insurance and $15,000 in Accidental Death and Dismemberment Insurance for all employees covered by this Agreement.
Life and AD&D Insurance. The City shall provide each employee with a paid $50,000 group term life insurance policy plus $81,000 of accidental death and dismemberment coverage and will pay one hundred percent (100%) of the premium.
Life and AD&D Insurance. The City will provide group life insurance and accidental death and dismemberment insurance equivalent to one-and-a-half times annual base salary to bargaining unit members beginning the first day of the month following the date of hire.
Life and AD&D Insurance. The Employer will offer to eligible full-time employees life and accidental death and dismemberment insurance in the amount of two (2) times an eligible employee’s annual salary at no cost. Employees have the option to purchase additional supplemental life and/or accidental death and dismemberment insurance coverage for themselves and/or their spouses and dependent children through payroll deduction. The Employer agrees to offer all insurance benefits on a pre-tax basis where allowable. Domestic partner benefit coverage will be offered to employees as required by law.
Life and AD&D Insurance. The City shall provide all full-time bargaining unit employees with fully paid Group Life and Accidental Death and Dismemberment coverage in the amount of two (2) times the employee's annual base salary. The City may offer employees the option to voluntarily purchase additional life insurance coverage (for self, spouse, child) through a provider of the City's choice, at the employee's own cost, after tax, and through payroll deduction.
Life and AD&D Insurance. County will continue to make the contributions necessary to provide employees with a group life and accidental death and dismemberment (AD&D) insurance in the amount of two (2) times the employee’s annual salary, or as amended by County.
Life and AD&D Insurance. Group Term Life Insurance coverage in the amount of $50,000 and Accidental Death and Dismemberment (AD&D) coverage in the amount of $50,000 shall remain in effect for Employees with the premiums for such insurance to be paid by the City. The City will pay the full premium for regular part-time Employees. Additionally, supplemental coverage shall be made available for purchase by Employees, with the amount, terms and conditions as specified by the insurance carrier.
