Incident Investigation. Centogene shall be responsible for the evaluation of reported Incidents. Centogene shall investigate any report of an Incident. Shire shall cooperate with, and provide reasonable and necessary information and assistance to, Centogene in connection with such investigation. Centogene shall immediately notify Shire of the results of any investigation related to Contract DBS Test Kits.
Incident Investigation. 3.7 Where a work-related death occurs, the police take primacy initially in the investigation in accordance with the Work Related Deaths Protocol (WRDP) and the associated Practical Guide. The police / Crown Prosecution Service (CPS) are responsible for the investigation and prosecution of certain offences. This includes individual or corporate manslaughter, ill treatment or wilful neglect and extends to cases of theft in an abuse case. HIW or HSE will notify them of such incidents and work with them in accordance with the WRDP and this MoU.

All incidents shall be investigated in conjunction with Eskom Rotek Industries in terms of OHS Act General Administrative Regulations 8 and 9, using Eskom Procedure 32-95 as a reference. The objective of incident investigation, should not only be a legal requirement, but should establish why and how the incident occurred and find out the root cause of the incident and to decide on precautionary measures that are required to address the root cause to prevent any further recurrences of the same or similar incidents.

Reviewing Customer’s networking diagrams to determine the design of the existing network infrastructure. Conducting onsite interviews with Customer’s representatives and designated Customer personnel responsible for: o Managing servers, clients, and remote systems to determine connectivity and management processes; o Internet gateway security to determine availability of solutions to provide information security protection, monitoring and mitigation; o Email security to determine availability of solutions to provide information security protection, monitoring and mitigation managing the endpoint security solutions to identify monitoring capabilities. Establishing procedures for documentation of actions taken and the handling of findings. Scheduling the necessary resources and establishing meeting cadence in coordination with Customer’s Project Manager.
Incident Investigation. Lessee shall conduct incident investigations to: • Prevent further possible injury and property damage • Collect facts about the incident • Prevent recurrence Root-cause analysis should be performed to determine the root cause of incidents. Incidents to include accidents/incidents, injuries, illnesses, and near misses, must be reported to the City representative as soon as possible. All accidents/incidents must be reported to the City representative as soon as possible. An initial written report must be completed within 24 hours or the next business day, whichever is earlier and submitted to the City representative. Accountability
Incident Investigation. The Joint Occupational Health and Safety Committee shall be notified by the Employer in a timely manner of each accident, incident and injury involving an employee which has occurred since the last meeting of the Committee. The Committee shall investigate the incident jointly, by one (1) representative of the Union and one (1) Employer representative and report to the Union and the Employer on the nature and cause of the accident or injury. In the event of a fatality, the Employer shall immediately notify the Union designate.

Incident Investigation. The Contractor is required to investigate all incidents and provide an incident report to Sydney Water’s Contract Manger. Sydney Water’s Contract Manager will be notified of investigations and will participate in the Contractor’s investigations at his/her discretion. Sydney Water may also conduct its own investigations of serious incidents and the Contractor must cooperate in this process. The Contractor’s incident investigation must meet the intent of the key requirements of Sydney Water’s incident investigation procedure. The Contractor must appoint a suitably qualified, competent and independent incident investigation leader and investigation team, based on the incident classification level. The Contractor must inform Sydney Water’s Contract Manager in writing of the name of the appointed incident investigation leader. If Sydney Water’s Contract Manager reasonably objects to the person appointed, the Contractor must replace that person.
