The Application Response will include, at a minimum, the configuration of the space, the Cable Installation Fee, Cable Records Fee, and the space preparation fees, as described in Section 8.
In Louisiana. Cancellation of This Agreement, # 4 is replaced with the following: We reserve the right to cancel this Agreement upon thirty (30) days written notice, at Your last known address. However, in the event of customer fraud, material misrepresentation, failure to pay, or termination as a customer, cancellation may be immediate. The notice of cancellation will include the reason and the effective date of cancellation.

Examples of extraordinary conditions include but are not limited to, extended license or permitting intervals; major BellSouth equipment rearrangement or addition; power plant addition or upgrade; major mechanical addition or upgrade; major upgrade for ADA compliance; environmental hazard or hazardous materials abatement; and arrangements for which equipment shipping intervals are extraordinary in length. The Parties may mutually agree to renegotiate an alternative provisioning interval or BellSouth may seek a waiver from this interval from the Commission.

In Louisiana. ▇▇▇▇▇▇-▇▇▇▇▇▇▇▇▇▇▇ & Associates, Inc. (SKA) was contracted to provide topographic survey services and preliminary and final roadway and bridge design services to widen I-10 from a four- lane freeway section to a six-lane freeway section. The roadway section is approximately 4.5 miles long and involves removing the inside shoulder and widening to the inside with a new 12’ travel lane and 10’ inside shoulder, with center barrier rail where median widths are narrow. The bridge design services include the bridge superstructure replacement of the overpasses at LA 429 and LA 30, as well as the bridges at Bayou ▇▇▇▇▇ including hydrologic/hydraulic analyses, and full replacement (substructure and superstructure) for the LA 73 interchange to accommodate for future LA 73 improvements. The overall project corridor is nearly 16 miles and is being built to widen interstate 10 from Highland Road to LA 22 in East Baton Rouge and Ascension Parishes. SKA is providing project management, as well as design services for all phases of this project, and is working closely with CDCs of adjacent project to ensure corridor continuity. In addition, at the request of LADOTD, SKA had investigated the feasibility of implementing an innovative interchange design in the form of a Single Point Urban Interchange (SPUI) at LA 73.

The Louisiana State Police is designated as the CJIS Systems Agency (CSA) for Louisiana, and as such is responsible for ensuring all entities, public or private, protect Criminal Justice Information (CJI) accordingly. By executing this agreement with the LSP, the vendor is electing to participate in the LSP’s Centralized Vendor Vetting Program (CVVP) and comply with the requirements set by the CJISSECPOL and the state of Louisiana. The CVVP will assess the technical compliance of the vendor’s proposed product and/or service. Additionally, all vendor personnel supporting the contract will be screened for suitability in accordance with the CJIS Security Policy. CVVP Approved Vendors will be required to maintain compliance with technical and personnel requirements established in this agreement. The LSP will review approved vendor’s oIerings and personnel annually and reassess on a triennial basis. This agreement is independent from all contracts between the vendor and client CJAs. Execution of this agreement does not constitute purchase, or intent of purchase, of any vendor oIering by the state of Louisiana or the Louisiana State Police.
