Catastrophic Illness/Injury. A devastating illness or injury that is expected to incapacitate the employee or a member of the employee’s immediate household family for an extended period of time, provided that taking extended time off from work creates a financial hardship for the employee because the employee has exhausted all leave balances. Catastrophic is defined as an illness or injury which results in the employee or the employee’s immediate family member requiring a level of care and treatment beyond what he or she could provide without assistance. Catastrophic illness or injury would be defined as any life-threatening illness or injury that would require the employee to be off the job for a minimum of 180 calendar days.

Donation – the act of voluntarily, unconditionally, and irrevocably surrendering a portion of one’s sick leave, vacation leave, personal leave, and/or compensatory time to a qualified employee. Immediate Family Member - Mother, father, spouse, son, daughter, stepson, stepdaughter, legal guardian, or someone who legally stands in place of a parent.
Catastrophic Illness/Injury. When an employee has accrued the maximum accumulation of sick leave as set forth, thereafter 50% of the additional sick leave earned and not used (while such maximum level is maintained) will be credited to the employee's catastrophic illness/injury sick leave reserve account. During the last three (3) years of employment, an employee who has, at the time of discontinuance of employment, met the minimum qualifications for a retirement annuity from the Wisconsin Retirement Fund or who qualifies for a disability pension as defined by Chapter 41, Wis. Stats., may draw from that employee's catastrophic illness/injury sick leave reserve account (if any) for any absence in excess of 80 hours during the annual period due to a catastrophic illness or injury (annual period being the employee's anniversary date of employment). Catastrophic illness/injury for purposes of this section is defined as an illness, disease, or injury which:
Catastrophic Illness/Injury. If an employee is diagnosed by a qualified Doctor of Medicine to have a catastrophic illness (in a state of total disability from cancer, heart attack, AIDS, etc.) or has a catastrophic injury (an injury that totally disables an employee for an extended period of time), the employee may be entitled to extended health benefit coverage (Medical, Dental, & Life ins.) provided by the City (in the same fashion normally provided to the employee). This benefit requires the approval of the Department Head and Personnel Officer, and is applicable when the employee's leave benefits are exhausted, under the following formula: Years of Full-Time City Employment Months of Continued Coverage After (2) years 3 months After 4) years 6 months After (7) years After (10) years 9 months 12 months

Xxxx Leave Donation Procedure A member of the OAPSE Local 581 who has exhausted all accumulated paid leave as a result of a catastrophic illness or injury of a temporary nature may be granted additional sick leave days through the donation of accumulated unused sick leave by other Bargaining Unit members who volunteer to do so in accordance with the following guidelines: For purposes of this procedure, the term "catastrophic illness or injury" shall include only those illnesses or injuries which are calamitous in nature, constituting a great misfortune.
