Waiver of Insurance Benefits. If the Insured fails to file a perfected Claim within the time required by subsection (c), but does file a perfected Claim within one (1) year after the applicable event described in subsection (a), the Company shall re-open and process such perfected Claim under this Policy; however, there shall be no further interest or Advances included in the Claim Amount for periods following the subsection (a) deadline to file the Claim. If the Insured fails to file a perfected Claim within the one (1) year period specified, such failure shall be deemed an election by the Insured to waive its right to any Insurance Benefit under this Policy with respect to such Covered Loan. Waiver of Insurance Benefits. If the SBA/BS waives the benefits, he will be allowed to reinstate them if there 'is a life altering event during the year. Also, he will be able to reinstate these benefits during open enrollment. The SBA/BS will be paid $2,500 for waiver of benefits.
Waiver of Insurance Benefits. Members may choose to waive or ‘opt-out’ of insurance benefits. Members choosing to ‘opt-out’ will be required to sign a release indicating that their dependents are covered under another health benefit program. Members shall be told how to re-enroll in the health benefits program if needed. Members are responsible for informing the school business administrator of any changes in circumstances regarding health benefits. Employees who are not employed during the full year (Sept. 1 – Aug. 31) and who choose the waiver option shall have their payments prorated accordingly. This applies to new hires after September 1st and any employment termination that is effective prior to August 31st. Employees who choose the waiver and are on an unpaid leave of absence without health benefits shall have their payments prorated as well. The Board shall pay members who waive their insurance benefits as follows: $2,000.00 for opt out of single coverage; $2,500.00 for opt out of parent/child(ren) coverage; $3,000.00 for opt out of husband/wife coverage; and $3,500.00 for opt out of whole family coverage.
Waiver of Insurance Benefits i. Where, as of the Effective Date, a HUD/FHA-insurance claim has not been submitted to HUD, no claim for insurance shall be submitted by FTB. FTB must observe all HUD requirements for servicing and payment of mortgage insurance premiums with respect to such mortgages.
Waiver of Insurance Benefits. Food Service Department employees eligible for benefits may elect to waive all individual coverage for the health and dental insurance, and/or the Life Insurance. Employees will receive a payment of $500 for waiving the health and dental and $471.50 for waiving health insurance only. Payment made to employees waiving coverage will be made on a semi-annual basis prior to the first pay period in January and June. Notice of intent to elect the waiver must be filed by June 15 of the previous school year. The waiver shall remain in effect until the employee files a written request for reinstatement of insurance. When there is a change in the employee’s status (i.e., change in spouse’s employment or benefit program), the waiver may be revoked by written notice. Upon receipt of revocation of the waiver, coverage by the health and dental insurance carrier shall be subject to any regulations or policy restrictions, including waiting periods, which may then be in effect. Life Insurance coverage shall be subject to the underwriting provisions of the carrier to accept the application. Depending upon the effective date of insurance reinstatement, an appropriate adjustment will be made in pro-rating the amount of the waiver.
