Waiver of Insurance Benefits. If the Insured fails to file a perfected Claim within the time required by subsection (c), but does file a perfected Claim within one (1) year after the applicable event described in subsection (a), the Company shall re-open and process such perfected Claim under this Policy; however, there shall be no further interest or Advances included in the Claim Amount for periods following the subsection (a) deadline to file the Claim. If the Insured fails to file a perfected Claim within the one (1) year period specified, such failure shall be deemed an election by the Insured to waive its right to any Insurance Benefit under this Policy with respect to such Covered Loan. Members choosing to ‘opt-out’ will be required to sign a release indicating that their dependents are covered under another health benefit program. Members shall be told how to re-enroll in the health benefits program if needed. Employees who are not employed during the full year (Sept. 1 – Aug. 31) and who choose the waiver option shall have their payments prorated accordingly. Employees who choose the waiver and are on an unpaid leave of absence without health benefits shall have their payments prorated as well. The Board shall pay members who waive their insurance benefits as follows: $2,000.00 for opt out of single coverage; $2,500.00 for opt out of parent/child(ren) coverage; $3,000.00 for opt out of husband/wife coverage; and $3,500.00 for opt out of whole family coverage.

Life Insurance coverage shall be subject to the underwriting provisions of the carrier to accept the application. Depending upon the effective date of insurance reinstatement, an appropriate adjustment will be made in pro-rating the amount of the waiver.
