TUBERCULOSIS CLEARANCE. Contractor will prohibit any agent or employee of Contractor from entering a District school site until Contractor has submitted to a tuberculosis risk assessment. If tuberculosis testing is warranted, Contractor agent or employee shall not enter a school site until Contractor has received, for that agent or employee, the “certificate” described in California Education Code §49406(d), showing the agent or employee to be free from infectious tuberculosis and dated within the sixty (60) days prior to the agent’s or employee’s first entry onto a District school site and will require an updated “certificate” every four years thereafter while that employee is continuously employed by Contractor or that agent is continuously retained by, or otherwise represents, Contractor.

TUBERCULOSIS CLEARANCE. In accordance with California Education Code Section 49406, each credential candidate prior to assignment to Institution must obtain at the candidate’s sole expense an examination by a licensed physician or surgeon within the past 60 days to determine that they are free of active tuberculosis, prior to beginning the candidate’s assignment in the Institution.
TUBERCULOSIS CLEARANCE. The undersigned does hereby certify to the governing board of the District as follows: I am a representative of the Contractor currently entering into this Agreement with the District and I am familiar with the facts herein certified, and am authorized and qualified to execute this certificate on behalf of Contractor. Contractor’s responsibility for tuberculosis (“TB”) clearance extends to all of its employees, subcontractors, and employees of subcontractors coming into contact with District pupils regardless of whether they are designated as employees or acting as independent contractors of the Contractor. Contractor certifies that at least one of the following items applies to the Services that are the subject of the Agreement:  The Contractor ensures that any person providing any portion of the Services with more than limited contact with District students (as determined by the District) has, at no cost to the District, completed a TB risk assessment within the past 60 days, and, if risk factors are identified, has received a TB test in compliance with the requirements of Education Code section 49406. A complete and accurate list of Contractor's employees and of all of its subcontractors' employees who may come in contact with District pupils during the course and scope of the Agreement is attached hereto; and/or  Contractor shall only have limited or no contact (as determined by District) with District students at all times during the Term of this Agreement. TUBERCULOSIS CLEARANCE. Contractor shall certify in writing that Contractor’s employees, volunteers, and subcontractors receive clearance for TB. In such cases where Contractor does not have in-person contact with students, contractor shall not be required to obtain TB clearance.
TUBERCULOSIS CLEARANCE. Contractor will ensure that all of its employees who will be working on the District’s school sites with students are determined to be free of active tuberculosis by a medical professional. Contractor shall submit documentation to the District verifying that all of its employees who will be working on the District’s school sites are determined to be free of active tuberculosis.
TUBERCULOSIS CLEARANCE. Each family member that will be participating in the program is required to submit a negative t.b. clearance, dated within the last year. The family member who is enrolled in the parenting course is eligible to receive t.b. testing at MJC’s Health Services for a fee or you can pay this fee to your own private doctor or the Public Health Department.

If you should drop before the start of the semester, only the parenting course fee is refundable. Your child’s tuition for the program is not refundable. TUBERCULOSIS CLEARANCE. Pursuant to California Health and Safety Code Section 121545, District shall advise Students that an up-to-date tuberculosis clearance (negative skin test or a physician’s certificate of clearance) may be required by Facility and if so, must be provided to Agency prior to the start of any activities. Additional screening and instruction may be required by Agency prior to placement.
TUBERCULOSIS CLEARANCE. Contractor assures that its employees, student teachers, subcontractors and agents providing services to District students are adequately screened so as to prevent the assignment of personnel who may pose a threat to the safety and welfare of students, and that all such personnel shall provide evidence of freedom from tuberculosis for a period within six (6) months prior to the onset of service and provide annual certification thereafter.
