Required Report. Each unit member who has been on sabbatical leave shall file with the Human Resources Office a written report within sixty (60) days of the unit member's return to duty. Such report shall contain a detailed description as to the activities of the unit member, together with the unit member's appraisal of the professional value of the experience gained while on sabbatical leave, the manner in which such experience or knowledge gained may be used for the benefit of the students or the school in which the unit member is located, and any other data necessary for a satisfactory report and which contains transcripts of all college and university study while on leave.
Required Report. Within sixty (60) days of returning to academic duties at the University after a sabbatical leave, the faculty member shall submit a detailed report of activities during the leave to the Xxxxxxx.
Required Report. Within eight (8) weeks after the expiration of the 10 leave, the unit member shall submit in writing three copies of a detailed evaluation of his 11 activities while on leave, showing evidence that he/she has met the objectives stated in his/her 12 application.

Required Report. Within thirty (30) days after completion of the drilling required to exercise the Reacquisition Option, MPDP shall prepare and deliver to AMERA (i) drill logs of the drilling completed, and (ii) a thorough technical report of the data and interpretations derived from such drilling, including all assays and geochemical, geophysical and geological analyses made.

Required Report. Upon completion of the activity funded by the Grant, Recipient will provide Administrator with a report regarding the use of funds provided by Administrator. Acceptance of the grant and use of the funds is conditional upon Recipient’s completion of all information fields requested in online reporting required by Administrator, without modification. Recipient will provide the report using an online reporting system created by Administrator. The report will be due once the funded activity is complete or by July 15, 2014 at the latest. The report will describe attainment of objectives and outcomes, reconciliation of all expenses using required form and copy of all receipts, and other requested information as described in Administrator’s report request documents. If a report is not submitted, the Recipient will be ineligible to apply for grants in subsequent funding cycles. A summary of the report will be provided to Donor(s) and may be published by Administrator in electronic or print format. Portions of the report may be communicated to the orthopaedic community and Donor(s) in aggregate form with information provided by other grant recipients and may be published by Administrator in electronic or print format.

Such report shall contain detailed data as to the activities of the employee, together with the employee's appraisal of the professional value of the experience gained while on leave, the manner in which such experience or knowledge gained may be used for the benefit of the students or the school in which the employee is located, and any other data necessary for a satisfactory report. The employee shall not be considered as having completed the requirements of sabbatical leave until his/her report has been recommended by the sabbatical leave committee and approved by the Superintendent and the Board of Trustees.
