While particular rules, equipment and/or personal discipline may reduce the Risks, the Risks do exist, and I understand that these Risks may be caused in whole or in part by Xxxxx's own actions or inactions, the actions or inactions of others participating in the Event, or the negligent acts or omissions of the Event Organizers (defined below). The Risks include, but are not limited to, minor injuries such as bruises, cuts and scrapes and more serious injuries including concussions, sprains, strains, fractures, over- use syndrome, broken bones, dislocation of joints, muscle pulls and ligament/tendon damage.

Patients with a history of alcohol and/or drug abuse are at increased risk for developing addiction. Allergic reactions are possible with any medication. This usually occurs early after initiation of the medication. Most side effects are transient and can be controlled by continued therapy or the use of other medications. Failure to adhere to the above policies may result in cessation of your controlled substance prescribing by this provider or referral for further specialty assessment.
