Premium Rate. A Producer which does not qualify as a "$15 Million Contributor" shall contribute to the Health Plan:
Premium Rate. One and one-half (1½) times the employee’s regular rate.
Premium Rate. Premium rate of pay is 1-1/2 times the base rate of pay plus such other differentials that may apply. Hours worked at premium rates will be counted toward weekly overtime.
Premium Rate. For those employees specifically assigned to train or cross-train other employees for periods of four (4) or more hours, a premium of $.50 per hour for all such hours shall be paid.
Premium Rate. Essential personnel will be paid their regular rate of pay plus one and one half (1 ½) times their regular rate for the hours actually worked.
Premium Rate. Your Premium rate is determined using the following acceptable rating factors: age, tobacco use, family size, and geography. [Your][An Enrollee’s] tobacco use impacts the determination of [Your][the] Premium rate. Upon enrollment, You will attest to [Your][an Enrollee’s] tobacco use. If You report false or incorrect information to Us [or the Exchange] about [Your][an Enrollee’s] tobacco use upon enrollment, We may retroactively apply the appropriate tobacco rating factor to [Your][the] Premium as if the correct information had been accurately reported from the Effective Date for Coverage.
Premium Rate. The minimum premium rate paid in addition to his/her normal wage rate to a Journeyman Sheet Metal Worker when designated by management as a xxxxxxx shall be 13% of the base rate plus vacation pay.
Premium Rate. In the event that the employee is not qualified to perform all the duties, he/she will not receive the $1.20 premium rate.
Premium Rate. The monthly premium rate is set forth on the rate notification provided to the Group. In general, the premium is not pro-rated and there are no partial months of coverage. However, if agreed upon by the parties, if coverage will be issued starting on the 16th calendar day of the month or later, then premium for that month will be pro-rated. If for a Group renewal, or the State of Oregon or any other taxing authority imposes upon SHP any new or additional tax or license fee which is levied upon or measured by premium, by SHP’s gross receipts, or by any portion of either, then SHP may amend this Contract to increase the premium by an amount sufficient to cover all such taxes or license fees rounded to the nearest cent, effective as of the date stated in a notice sent to the Group. The effective date of such a premium increase shall not be earlier than the date of the imposition of such tax or license fee increase. SHP shall also have the right to change the premium as of any date that the extent or nature of the risk under this Contract is changed by amendment to this Contract or by reason of any change mandated by law of regulation.
