Health Insurance Benefit Program. All nurses in regular, established positions who are consistently scheduled for forty (40) or more hours per two-week period shall be eligible, as of the first day of the month following the first full month of employment, to participate in the health insurance benefit program offered by the Medical Center. Employees shall continue to be offered benefit options, in accordance with the terms of the program, with regard to medical, dental, vision, life, AD&D, long- term disability and short-term disability plans, critical illness insurance, accident insurance and healthcare and dependent care spending accounts. Medical and dental coverage shall continue to be extended to legally domiciled adults as defined in the health insurance benefit program.

The Medical Center shall forward to the Association any changes in the terms of the health insurance benefit program at least ninety (90) days prior to implementation and shall forward any changes in premium rates at least sixty (60) days prior to implementation.

All nurses who are regularly scheduled to 3 work at least twenty (20) hours per week are eligible to participate in the health 4 insurance benefit program offered by the Employer. Nurses shall be offered 5 benefit options, in accordance with the terms of the Employer’s program, with 6 regard to medical, dental, vision, life, AD&D, long-term disability and short-term 7 disability plans, and healthcare and dependent care spending accounts. The 8 Employer will maintain its current non-standard health insurance benefits for this 9 bargaining unit until December 31, 2014. Thereafter, the Employer will provide 10 for this bargaining unit its standard health insurance benefits in accordance with 11 the Professional Agreement between Sacred Heart Medical Center and the 12 Association.

New employees shall default into the PPO plan if they 14 do not select an insurance plan by the employer specified 15 enrollment deadline.

Nurses shall be 24 offered benefit options, including but not limited to, in accordance with the 25 terms of the Employer’s program, with regard to medical, dental, vision, 26 critical illness insurance, accident insurance life, AD&D, long-term disability 27 and short-term disability plans, and healthcare and dependent care spending 28 accounts.
