Employee Discount. The Employer will offer employees covered under Employer medical plans the most favorable discount for services rendered at PeaceHealth facilities, providers and laboratories.

To the extent Manager provides discounted rates to Manager’s employees at other hotels managed by Manager or its Affiliates, pursuant to discount rate programs applicable to other hotels, Manager agrees to include this Hotel in such discounted rate programs (subject to availability and black-out periods determined by Owner and Manager during the Annual Plan process, or as otherwise approved by Manager or Owner as part of the revenue management of this Hotel) and to provide the same discounted rates to the Hotel Employees, to the extent allowed under the management and franchise agreements affecting such other hotels.

Group Benefits Once eligible, participation is mandatory for all Group Benefits; however, opting out of Extended Health and/or Dental is permitted if an employee has proof of alternative coverage which must be provided during the enrolment process. Group benefit premiums (excluding Short-Term Disability) are based on the employee’s Base Pay for full-time employees, on a flat coverage rate for part-time employees, and on ABBR for commissioned employees. Premium costs are shared as outlined in Exhibit A – Benefit Programs and Eligibility. Employees are automatically assigned single coverage in the Extended Health and Dental plans upon eligibility. Employees wishing to cover their dependents must select family coverage, for Extended Health and/or Dental, during the 31-day enrolment period. If both and employee and his/her spouse work at Rogers, and if both are eligible for participation in the Group Benefits Program, both may apply for family coverage to maximize coverage for Extended Health and/or Dental benefits. You may make changes to your coverage within 31 days of a life event. If you do not complete the enrolment change process within 31 calendar days of the date of the life event you will require an application under the Late Entrant process which includes providing Evidence of Insurability.
