BASIC HEALTH CARE BENEFIT PLAN FOR ELIGIBLE EMPLOYEES AND DEPENDENTS. 4.01 The basic Heath Care Benefit PLAN shall be as described herein for eligible employees and their dependents: Extended Health Care Benefits for Employees and Dependents: Semi-private hospital accommodation, stay in licensed private hospital or nursing home, services of registered nurse or licensed practical nurse, services of a speech therapist, physiotherapist and clinical psychologist, rental or purchase of certain medical supplies and prosthetic devices including standard wheelchairs, hospital beds, oxygen equipment and other equipment usually found only in hospitals, hearing aids, ambulance s ervices, other specialized treatments, emergency out-of-country expenses (including reasonable and customary doctors' fees and semi- private hospital accommodation required as the result of a medical emergency up to the level that would be payable if the expense were incurred in Canada) Prescribed drugs with a pay-direct drug card, reimbursed to the amount of a generic equivalent, where available, and with a maximum of $7.00 for the dispensing fee, vision care (up to $250 effective March 1, 2001 and $275 effective March 1, 2003 for glasses, contact lenses or laser eye surgery for you and each eligible dependent in any twenty-four (24) consecutive months. For fertility drugs, charges shall be limited to a lifetime maximum of $15,000. For anti- smoking drugs, charges shall be limited to a lifetime maximum of $400. For each drug, the reasonable and customary charge is limited to the cost of the generic equivalent drug, where available, and a maximum dispensing fee of $7.00

  • Dependent Care Salary Reduction Plan The Employer agrees to maintain the current dependent care salary reduction plan that allows eligible employees, covered by this Agreement, the option to participate in a dependent care reimbursement program for work-related dependent care expenses on a pretax basis as permitted by federal tax law or regulation.

  • Post Retirement Health Care Benefit Employees who separate from State service and who, at the time of separation are insurance eligible and entitled to immediately receive an annuity under a State retirement program, shall be entitled to a contribution of two hundred fifty dollars ($250) to the Minnesota State Retirement System’s (MSRS) Health Care Savings Plan. Employees who have a HCSP waiver on file shall receive a two hundred fifty dollars ($250) cash payment. If the employee separates due to death, the two hundred fifty dollars ($250) is paid in cash, not to the HCSP. An employee who becomes totally and permanently disabled on or after January 1, 2008, who receives a State disability benefit, and is eligible for a deferred annuity under a State retirement program is also eligible for the two hundred fifty dollar ($250) contribution to the MSRS Health Care Savings Plan. Employees are eligible for this benefit only once.
