Parent Permission. The basic goal of the District is to provide students with a meaningful and positive educational experience as members of athletic teams. This pamphlet is designed to help both the participant and his/ her parent/guardian become aware of Fredonia Central School’s basic expectations. To be more successful, athletic teams, coaches and participants need positive support and cooperation from the parent/ guardian, and other community members. The attached Agreement Contract and Health Questionnaire must be properly completed and returned to the Health Office and then the student’s coach before the student may participate on a team.
