INTRODUCTION TO OUR SERVICES. This Agreement details the terms and conditions related to all users including but not limited to Members, Providers and Customers under the name of Food Dollar and governs your use of the Contents, Licensed Applications, Oputech Services and/or External Services. To use the Oputech Services, you need to possess compatible hardware, software (latest version recommended and sometimes required) and Internet access (fees may apply). Our Oputech Services performance may be affected by these factors. You can use the Oputech Services and access the Contents provided by Oputech and/or External Party. Oputech reserves all the intellectual property rights of all the Contents. You are not allowed to archive, retrieve, copy, duplicate and/or share any of the Contents without the written approval from Oputech. All Contents, including materials and information provided by Oputech and External Party are not verified. The Contents are only for the sharing among all Oputech users as basis for discussion and quick reference and shall not be considered as advertisements, parts of agreements, and/or invitations to purchase any products or services. You have the full responsibility to independently verify the accuracy, consistency, reliability and quality of all the Contents before use. You shall always consult professionals and seek legal advice before entering any agreements or contracts. Oputech shall not be liable for any of your losses and damages as a result of the use of any Contents, Licensed Applications, Oputech Services and/or External Services. You also agree to indemnify Oputech as a result of, or incidental to, your act(s) or infringement of any intellectual property rights of any materials on indemnity basis. If you are a Provider, you may create and submit materials for the Oputech Services. Before submitting any materials, you must ensure that you own the intellectual property rights of the materials and are authorized to make the submission. You may be held responsible for any infringement of intellectual property rights and other legal obligations for your submission. When you make your submission, you irrevocably agree to allow Oputech to make use of the materials for whatever purposes deemed appropriate by Oputech. To use our Services, you need compatible hardware, software (latest version recommended and sometimes required) and Internet access (fees may apply). Our Services' performance may be affected by these factors. USING OUR SERVICES UNIFORMD is an application that allows you to list items for sale. INTRODUCTION TO OUR SERVICES. This Agreement governs and regulates the Customer’s hire of the Equipment (as hereinafter defined) from the ICTZV through ICTZV’s website (“Website”). Please read and understand this Agreement carefully before continuing to use the services and/or products provided by ICTZV on its Website. If the Customer does not understand this Agreement, or any services or products that the ICTZV provides or offer, the Customer should contact ICTZV before placing any order for hire, filling any forms and/or sending ICTZV any information. ICTZV reserves the right, in its sole and absolute discretion, to modify, vary, amend, change and/or update this Agreement, and any checklists or agreements which are incorporated herein at any time as it deems fit and with reasonable notice to the Customer, for example, by posting an update on the Website, or by emailing the updated Agreement to the Customer. Such modifications, variations, amendments, changes and/or updates to this Agreement shall be effective upon the posting of an updated version on the Website. The Customer agrees that it shall be its responsibility to review this Agreement regularly to ensure its understanding of this Agreement is current and understand the terms and conditions that apply to its hire of the Equipment. The Customer’s non-termination or continued use of the Equipment after the effective date of any modifications, variations, amendments, changes and/or updates constitutes the Customer’s acceptance of, and agreement to be bound by, the revised Agreement. If the Customer does not agree to this Agreement or with any subsequent modifications, variations, amendments, changes and/or updates, the Customer may terminate this Agreement in accordance with Clause 10.2 of this Agreement. If the Customer have any question regarding this Agreement, the Customer is advised to consult its professional lawyer.

  • Our Services As insurance intermediaries we generally act as the agent of our client. We are subject to the law of agency, which imposes various duties on us. However, in certain circumstances we may act for and owe duties of care to other parties, including the insurer. We will advise you when these circumstances occur, so you will be aware of any possible conflict of interest. We offer a wide range of products and services which may include: • Offering you a single or range of products from which to choose a product that suits your insurance needs; • Advising you on your insurance needs; • Arranging suitable insurance cover with insurers to meet your requirements; • Helping you with any subsequent changes to your insurance you have to make; • Providing all reasonable assistance with any claim you make. In some cases, we act for insurers under a delegated authority agreement and can enter into insurance policies, issue policy documentation and/or handle or settle claims on their behalf. Where we act on behalf of the insurer and not you, we will notify you accordingly and in relation to claims we will advise you of this fact when you notify us of a claim. Notwithstanding this, we endeavour to always act in your best interest. As intermediaries, we offer a wide range of insurance products and have access to many leading insurance companies and the Lloyd’s market. Depending on the type of cover you require and where we have provided advice based on a personal recommendation, we will offer you a policy from either: • a single insurer; • a limited range of insurers; or • a fair analysis that is representative of the insurance market. We will advise you separately as to which of these apply before we arrange your policy and where we have not undertaken a fair analysis of the market, we will provide you with a list of insurers considered. For all other policies, including optional additional products and premium finance (if relevant), before the insurance contract is concluded and after we have assessed your demands & needs, we will provide you with advice and make a personal recommendation. This will include sufficient information to enable you to make an informed decision about the policy that we have recommended, together with a quotation which will itemise any fees that are payable in addition to the premium. This documentation will also include a statement of your demands and needs. You should read this carefully as it will explain reasons for making the recommendation we have made.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits. For information about telemedicine services, our designated telemedicine service provider, and how to access telemedicine services, please visit our website or contact our Customer Service Department.

  • Customer Services Customer Relationship Management (CRM): All aspects of the CRM process, including planning, scheduling, and control activities involved with service delivery. The service components facilitate agencies’ requirements for managing and coordinating customer interactions across multiple communication channels and business lines. Customer Preferences: Customizing customer preferences relative to interface requirements and information delivery mechanisms (e.g., personalization, subscriptions, alerts and notifications).

  • Autism Services This plan covers the following services for the treatment of autism spectrum disorders. • Applied behavior analysis when provided and/or supervised by an individual licensed by the state in which the service is rendered. See the Summary of Medical Benefits for the amount that you pay. • Physical therapy, occupational therapy, and speech therapy services when rendered as part of the treatment of autism spectrum disorder. A benefit limit will not apply to these services. • Psychological and psychiatric services, and prescription drugs are also covered. See Behavioral Health Services and Prescription Drugs and Diabetic Equipment or Supplies for additional information. Coverage for autism spectrum disorders does not affect any obligation of a school district, a state or other governmental entity to provide services to an individual under an individualized family service plan, an individualized education program, or similar services required under state or federal law. Services related to autism that are furnished by school personnel are not covered under this plan.

  • Out of Scope Services Axon is only responsible to perform the professional services described in the Quote and this Appendix. Any additional professional services are out of scope. The Parties must document scope changes in a written and signed change order. Changes may require an equitable adjustment in the charges or schedule.

  • Conversion of Wholesale Services to Network Elements or Network Elements to Wholesale Services Upon request, BellSouth shall convert a wholesale service, or group of wholesale services, to the equivalent Network Element or Combination that is available to Global Dialtone pursuant to Section 251 of the Act and under this Agreement or convert a Network Element or Combination that is available to Global Dialtone pursuant to Section 251 of the Act and under this Agreement to an equivalent wholesale service or group of wholesale services offered by BellSouth (collectively “Conversion”). BellSouth shall charge the applicable nonrecurring switch-as-is rates for Conversions to specific Network Elements or Combinations found in Exhibit A. BellSouth shall also charge the same nonrecurring switch-as-is rates when converting from Network Elements or Combinations. Any rate change resulting from the Conversion will be effective as of the next billing cycle following BellSouth’s receipt of a complete and accurate Conversion request from Global Dialtone. A Conversion shall be considered termination for purposes of any volume and/or term commitments and/or grandfathered status between Global Dialtone and BellSouth. Any change from a wholesale service/group of wholesale services to a Network Element/Combination, or from a Network Element/Combination to a wholesale service/group of wholesale services, that requires a physical rearrangement will not be considered to be a Conversion for purposes of this Agreement. BellSouth will not require physical rearrangements if the Conversion can be completed through record changes only. Orders for Conversions will be handled in accordance with the guidelines set forth in the Ordering Guidelines and Processes and CLEC Information Packages as referenced in Sections 1.13.1 and 1.13.2 below.

  • Access to Services 1. Subject to DHCS provider enrollment certification requirements, the Contractor shall maintain continuous availability and accessibility of covered services and facilities, service sites, and personnel to provide the covered services through use of DMC certified providers. Such services shall not be limited due to budgetary constraints.

  • Billing Services Manager shall provide, or cause to be provided, the following billing services to P.C.:

Customer’s remedies for breach of this warranty are in the SLA. The remedies above are Customer’s sole remedies for breach of the warranties in this section.   • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.
