Green Shield. An extended Health Care Plan with the Employer paying one hundred (l00) percent of the premium which includes: - semi-private coverage - vision benefit of two hundred (200) dollars every twenty-four (24) months – no deductible - one (1) optometry exam per twenty-four (24) month period (sixty-five {65} dollars maximum) - extended health (eighty {80} percent for prescription drugs) The Employer agrees to provide equivalent coverage as per the Collective Agreement but reserves the right to present proposals to change carriers (or to self- insure). The change of carriers shall be subject to mutual agreement between the parties. A dispute over "equivalent coverage" may be referred to a single arbitrator for resolution.

Effective February 1, 2009, the eligible expenses outlined in the vision care coverage under this plan will include once per lifetime for laser eye surgery. Effective February 1, 2009, hearing aids $1,500 every thirty-six (36) consecutive months. Reimbursement provided through provider paid plan and/or mail order reimbursement for prescription drugs. Physiotherapy - $15.00 per visit, no limit. Massage - $15.00 per visit to a maximum of 12 visits per calendar year.
