Extended Health, Dental and Group Life Benefits. (a) The benefits and plans of insurance referred to in this section are qualified in their entirety by reference to the underlying policies and contracts of insurance or statutes or regulations. The Master Contract shall be controlling in all matters.

Extended Health, Dental and Group Life Benefits. The current practice of the Employer with regard to the Extended Health Care Plan, the Dental Plan, and the Group Life Plans outlined in “Taking Care - The Salvation Army Employee Benefit Plan” booklet shall continue for the term of this Collective Agreement. The Employer shall pay the monthly "Single” premium cost of the Core Plan for Health Care, Dental Care, Basic Life Insurance, and Accidental Death and Dismemberment for regular permanent full time employees working twenty five (25) or more regularly scheduled hours per week, following completion of the three month waiting period.

The master contract shall be controlling in all matters. The current practice of the Employer with regard to benefits and cost-sharing outlined in the "Taking Care" benefits booklet is acceptable and shall continue for the term of this agreement. It is agreed the Employer may change carriers and plans from time to time without penalty provided that any new package of benefits plans will provide for an option which is equivalent to the then current benefits. The Employer will provide a copy of the new plan. Where applicable premium costs should include coverage for the employee’s spouse and for dependants.
Extended Health, Dental and Group Life Benefits. The Employer will provide an Extended Health Care Plan, a Dental Plan, and a Group Life Plan. The benefits will include: Extended Health Benefits, Accidental Death and Dismemberment benefits; Dental Care Benefits; Group Life insurance benefits, and Dependant Life Insurance benefits as outlined in the current Healthcare Benefit Trust plan # 382. The Employer shall pay seventy-five percent (75%) of the monthly "Single, couple, or family" premium cost of the Plan for Extended Health Care, Dental Care, Group and Dependant Life Insurance, and Accidental Death and Dismemberment for regular employees working twenty-five (25) or more hours per week. The Employer will provide all eligible employees and the Union with a handbook detailing the above noted benefits. The Employer will not change the benefit carrier or current benefit plan without mutual agreement of the Union. Should the Employer want to change the benefit plan or benefit carrier the new benefit plan must cover all of the benefits outlined in the current Healthcare Benefit Trust Plan # 382 at the same or greater levels. Employees are not entitled to dual coverage of the benefits in Article 25.2.

The Employer shall pay the monthly "Single or couple" premium cost of the Core Plan for Health Care, Dental Care, Basic Life Insurance, and Accidental Death and Dismemberment for permanent employees working twenty-five (25) or more regularly scheduled hours per week.
