Eligibility Waiting Period. Employees who have been regularly employed in the District for more than thirty (30) continuous days are eligible on the first day of the month following 30 days of continuous regular benefit eligible service for District contribution to premium cost for health and life insurance provided herein.

Group – Any company, partnership, association, corporation or other legal entity which has made application for coverage herein and has agreed to comply with all the terms and requirements of this Benefit Plan. For the purposes of this Benefit Plan, the Group is the policyholder. Informal Reconsideration – A request by telephone for additional review of a utilization management determination not to authorize. Informal Reconsideration is available only for initial or Concurrent Review determinations that are requested within ten (10) days of denial. Investigational – A dental treatment, service, procedure, drug, device, or biological product is Investigational if the effectiveness has not been clearly tested and it has not been incorporated into standard dental practice. Any determination the Plan makes that a dental treatment, procedure, drug, device, or biological product is Investigational will be based on a consideration of the following:

Eligibility Waiting Period. Employees who have been employed in the District for more than thirty (30) days are eligible for health insurance benefits on the first day of the month following 30 days of continuous hire to a regular benefit eligible position and will be included in the insurance program for District employees and any supplemental or replacement program required to provide benefits described in Section 1, Active Employee Health Insurance. Insurance benefits provided to eligible employees include hospital, surgical, medical, major medical, dental, long-term disability, and life insurance coverage. Employees may enroll in optional or voluntary Insurance Benefits, i.e. vision, optional life, dependent life, accidental death, short term disability, after thirty (30) days of continuous regularly appointed service in the District in a benefit eligible position.
