Bed Hold Policy. If you reside in an assisted living facility or nursing facility, there may be a time when you need a higher level of care. If you are in an assisted living facility and need to stay in a nursing facility or hospital, Saint Xxxxxx XXXX’x policy is to hold your bed in the assisted living facility for 10 days or until the IDT determines it appropriate for you to return to your residence. If you are in a nursing facility and need a hospital stay, Saint Xxxxxx XXXX’x policy is to hold your bed in the nursing facility for 10 days, or until the IDT deems it appropriate for you to return to the nursing facility. If you do not return to your original place of residence within 10 days, we will make every effort to arrange return placement at the original assisted living facility or nursing facility. AFTERHOURSANDEMERGENCYCARE There may be times when you need to speak with a Primary Care Provider to receive advice or treatment for an injury or the start of an illness that can’t wait until regular Saint Xxxxxx XXXX center hours. Saint Xxxxxx XXXX provides 24-hour medical care. There is always a provider available 24 hours a day, 7 days a week, 365 days a year. When it is necessary to reach a Saint Xxxxxx XXXX provider and you do not believe it is an emergency, call Saint Xxxxxx XXXX at 000-000-0000. When the center is open, the clinic staff will connect you with a provider. When the center is closed, the answering service will call a provider for you. Please keep in mind that the Saint Xxxxxx XXXX provider returning your call may not be your personal Saint Xxxxxx XXXX provider, but he/she has been chosen by your doctor to answer your after-hours calls and is well-qualified to give you the care you need. EMERGENCY SERVICES Saint Xxxxxx XXXX covers emergency care for an emergency medical condition. An emergency is a life-threatening medical condition. If not diagnosed and treated immediately, emergent medical conditions could result in serious and permanent damage to your health. Bed Hold Policy. Rim Country Health and Retirement Community will assure that the resident/family/legal representative will receive written information related to the facility bed-hold policy at the time of admission, prior to transfer to another medical facility, before allowing the resident to go on a therapeutic leave and at the time any change is made under the State plan or facility policy. The information provided will specify the duration of the bed-hold policy under the State plan, if any, during which the resident is permitted to return to the facility and the nursing facility policy regarding bed-hold periods and guidelines. Guidelines: • Upon admission to the facility the resident/family/legal representative facility staff will review the bed-hold policy. • A copy of the bed-hold policy will be attached to the transfer form so that it will be sent with the paperwork at the time of transfer. A copy of both will be placed in the resident’s medical record. • The facility bed-hold policy will apply to all residents. • In the case of emergency transfer, when the written policy is not sent with the resident, a written notice to the family or legal representative will be provided within 24 hours of the transfer. • Bed-hold days of absence in excess of the State bed-hold limit that is determined by the individual payer are considered non-covered days, which means that the resident could use his/her own income to pay for the bed-hold. • Non-Medicaid residents will be requested to pay for all days of bed-hold. • Any resident whose hospitalization or therapeutic leave exceeds the bed-hold period under the State plan, will be readmitted to the facility immediately upon the first availability of a bed in a semi-private room if the resident requires the services provided by the facility and if the resident is eligible for Medicaid nursing facility services. • Medicaid-eligible residents will be admitted to the first available bed even if the resident has an outstanding Medicaid balance. • Once readmitted, these residents with an outstanding balance may be transferred if the facility can demonstrate that non-payment of charges exists and documentation and notice requirements are followed. Bed Hold Policy. The VMH agrees to reserve or “hold” a bed for a Short-Term Stay Resident who is admitted to a hospital or another health care facility offering a higher level of care, as long as the Short-Term Stay Resident or their Representative continues to pay the VMH at the current daily care rate to “hold” the bed.
