30 p.m if my child is left at the center after closing. I agree to follow the time stated on the in/out computer. I understand that after 6pm Monday through Friday, if no emergency person/relative can be located, campus security will be called and the proper authorities contacted. I agree to pay a $30.00 fee for a non-sufficient fund check and will pay for any returned check(s) with cash. I agree to accompany my child or children into the building and escort them to their classroom area. NO CHILD IS ALLOWED to walk down the stairs without adult supervision. I agree to sign my child in and out every day. I will notify the Director of the Child Care Center or the person in charge if someone other than myself will be picking up my child. I will provide either verbal or written communication to the center on who will be picking up my child. I will also provide contact information on the person picking up my child and an alternate contact number where I can be reached. If I dis-enroll my child, I understand that my child(ren)’s spot will be filled immediately from the center’s waiting list. In order for him/her to be readmitted to the child care center an opening will have to be available and my child will be placed on the child care center’s waiting list. I am the recipient of the Parent Handbook, Emergency Response Plan and Licensing Brochure and will comply with additional policies not mentioned above but included within that handbook. I understand that child care tuition may be subject to change during my child’s time of enrollment. I understand that I will need to sign a new contract each time my child care tuition changes. In an emergency, the center has my permission to seek medical help as it sees fit in my child’s best interest. I understand that all emergency information provided in my child’s file must be current and up-to-date. I understand that the center can only dispense medication with a physician’s written permission and the medication chart is signed with the correct information & a signed medication permission form is in my child’s file. This includes all mediations, including over-the-counter medicines. All medication must be labeled with your child’s name. Please note: an additional doctor’s note will be required for my child’s file for the child care center staff to administer the use of nebulizers, epi-pens and inhalers.   • Filing a Grievance Grievances may be filed by the Union on behalf of an employee or on behalf of a group of employees. If the Union does so, it will set forth the name of the employee or the names of the group of employees.

  • Step Five The Union can appeal the decision of the Secretary, or designee, within thirty (30) days to fact finding. When fact-finding is invoked, the Union and the Employer shall jointly request a list of seven (7) neutral fact-finders from the FMCS. The parties will meet within fifteen (15) days of receipt of the FMCS list to seek agreement on one of the listed fact-finders. This meeting may take place on the telephone. If the parties cannot agree on a fact-finder, the Employer and the Union will alternately strike one name from the list until a single name remains. A flip of the coin shall determine who shall strike the first name. The fact-finder shall resolve all questions related to the procedure. Upon mutual agreement of the parties, threshold issues may be resolved prior to the parties proceeding with the substantive issues involved in the case. The cost of the fact-finder shall be shared equally by the parties.

  • Labor Code Notice 2.6.1 All A-E and subcontractors must comply with the requirements of California Labor Code 1770 et seq. if the work performed is considered a “public works” under California Labor Code 1720 et seq. A-E is encouraged to contact the California Department of Industrial Relations for clarification if the A-E is unsure if some or any of the work performed under this CONTRACT qualifies as “public works”.

  • No Release; Return or Destruction Each Party agrees not to release or disclose, or permit to be released or disclosed, any information addressed in Section 6.9(a) to any other Person, except its Representatives who need to know such information in their capacities as such (who shall be advised of their obligations hereunder with respect to such information), and except in compliance with Section 6.10. Without limiting the foregoing, when any such information is no longer needed for the purposes contemplated by this Agreement or any Ancillary Agreement, and is no longer subject to any legal hold or other document preservation obligation, each Party will promptly after request of the other Party either return to the other Party all such information in a tangible form (including all copies thereof and all notes, extracts or summaries based thereon) or notify the other Party in writing that it has destroyed such information (and such copies thereof and such notes, extracts or summaries based thereon); provided, that the Parties may retain electronic back-up versions of such information maintained on routine computer system backup tapes, disks or other backup storage devices; provided further, that any such information so retained shall remain subject to the confidentiality provisions of this Agreement or any Ancillary Agreement.

  • Cooling Off Period You may terminate this Contract with immediate effect at any time within the period of 14 days after the date this Contract is entered into without incurring any liability to DFMC.

  • Servicemembers Civil Relief Act The Mortgagor has not notified the Seller that it is requesting relief under the Servicemembers' Civil Relief Act, and the Seller has no knowledge of any relief requested or allowed to the Mortgagor under the Servicemembers' Civil Relief Act.

  • Response Times (a) LogRhythm will respond to new support cases whether received via a telephone call or email within (i) four (4) Support Hours after receipt if received during a Business Day or (ii) by 12:00 p.m. Mountain Time the following Business Day if received after the end of a Business Day. LogRhythm will respond to new support cases via email or by directly contacting the applicable designated users. Response times for open support cases will vary depending on the specifics of the case and any Escalation required. If a response will require more than one business day to prepare, Customer will be notified and informed when a response can be expected.

  • Form 8-K The Company shall, on the date hereof, retain its independent public accountants to audit the financial statements of the Company as of the Closing Date (the “Audited Financial Statements”) reflecting the receipt by the Company of the proceeds of the Offering and the Private Placement, as well as the proceeds from the exercise of the Over-Allotment if such exercise has occurred on the date of the Prospectus. Within four (4) Business Days of the Closing Date, the Company will file a Current Report on Form 8-K with the Commission, which Report shall contain the Audited Financial Statements.

  • REPORT SUBMISSION 1. Copies of reporting packages for audits conducted in accordance with 2 CFR Part 200, Subpart F-Audit Requirements, and required by PART I of this form shall be submitted, when required by 2 CFR 200.512, by or on behalf of the recipient directly to the Federal Audit Clearinghouse (FAC) as provided in 2 CFR 200.36 and 200.512

  • Early and Safe Return to Work The Hospital and the Union both recognize their obligations in facilitating the early and safe return to work of disabled employees.
