The Student Documentation clause outlines the requirements and responsibilities regarding the collection, maintenance, and provision of records related to students. Student Documentation. School will maintain all documentation required to evidence compliance by each Program Participant with the terms and conditions of Subsections 1(g)-
Student Documentation. The Program shall maintain student documentation to support eligibility as specified in Section C. and enrollment as specified in Section J. The Program shall, on behalf of the District, request school records for each student from the last school they attended. The Program shall maintain documentation of case management, student assessment, basic skills gains, attainments of credentials, earned indicator of academic progress, and award of credit. The Program will comply with all state and federal laws related to the privacy, sharing, and retention of student records. Access to all student records will be provided in accordance with the Family Educational Rights and Privacy Act (FERPA). Monthly Student Reporting: The District will ensure that all required Program student information is reported in the student information system; and in CEDARS in accordance with OSPI’s standard procedures. The District will work with the College to determine whether District or the Program staff will be responsible for performing required data entry following OSPI’s standard procedures for all Reengagement Programs. If the Program is responsible for data entry, the District will provide access to the student information system, as well as, training and technical assistance. If the District is responsible for data entry, the District will define the data elements the Program must provide for each student, as well as, the format and required reporting dates for the submission of data.
Student Documentation a) Graduation Alliance shall maintain student documentation to support eligibility as specified in Section C. and enrollment as specified in Section F.
b) Graduation Alliance shall, on behalf of the District, request school records for each student from the last school they attended.
c) Graduation Alliance shall maintain documentation of award of credit.
d) Graduation Alliance will comply with all state and federal laws related to the privacy, sharing, and retention of student records.
e) Access to all student records will be provided in accordance with the Family Educational Rights and Privacy Act (FERPA).

Student Documentation. Dallas ISD shall ensure that all Dallas ISD students receiving prekindergarten services in Dallas ISD facilities shall be current on all immunization requirements, and shall complete the following Dallas ISD documents or provide the following documents: Dallas ISD enrollment form; home language survey; birth certificate; address verification (e.g., gas, electricity, or water ▇▇▇▇, lease agreement, or mortgage statement); income verification (income tax return or current paycheck stub); parent/guardian photo identification; and current immunization record. Dallas ISD shall share enrollment data with the Provider to streamline the enrollment process in prekindergarten and Head Start.

d) Graduation Alliance will comply with all state and federal laws related to the privacy, sharing, and retention of Student records and with Exhibit C, an executed Data Security Agreement with District.
e) Access to all Student records will be provided in accordance with the Family Educational Rights and Privacy Act (FERPA) For the purposes of FERPA, Graduation Alliance shall be considered a “school official” to the extent that providing documentation to Graduation Alliance is necessary for the purposes of the Program.

e. Access to student records will be provided in accordance with the Family Educational Rights and Privacy Act (FERPA) and Ohio law.
