Pharmacy and Therapeutics Committee. The Contractor shall utilize a Pharmacy and Therapeutics Committee (P&T) in accordance with KAR Title 907. The P&T shall meet in Kentucky periodically throughout the calendar year as necessary and make recommendations to the Contractor for changes to the PDL or drug formulary. The P&T shall be considered an advisory committee to a public body thereby making it subject to Kentucky’s Open Meetings Law found in KRS 61.800 to 61.850. Prior to each new calendar year, the Contractor shall give notice to the Department of the time, date and location of the P&T meetings. The Contractor shall make every reasonable effort to ensure that meeting dates and times for the P&T do not conflict with the meeting times for other MCO P&T or FFS P&T meetings. This shall be for the purpose of allowing attendance and travel for interested parties and the Department’s pharmacy staff. Final decisions are to be posted and maintained on the Contractor’s pharmacy information webpage and/or web portal.
Pharmacy and Therapeutics Committee. The Pharmacy and Therapeutics (P&T) Committee meets at least quarterly and is facilitated by the Pharmacy Provider. The Provider Chief Medical Officer (CMO) and Chief Nursing Officer (CNO) must participate in this meeting.
Pharmacy and Therapeutics Committee. The State will be entitled to have a representative from the State attend meetings of the Pharmacy and Therapeutics Committee and Pharmacy Benefit Manager will provide MMB minutes of such meetings and/or periodic reports as may be prepared by the Pharmacy and Therapeutics Committee as soon as reasonably possible, upon request, to the extent such minutes or reports specifically relate to services provided under this Agreement. Pharmacy Benefit Manager will redact any Pharmacy and Therapeutics Committee meeting minutes to preserve the confidentiality of the identity of the committee members. Attendees at Pharmacy and Therapeutics Committee Meetings must execute a confidentiality agreement related to such attendance and any minutes or meeting reports from such meetings must be maintained by the State as confidential and restricted from further disclosure, including pursuant to a public records request, to the maximum extent permitted by law.
Pharmacy and Therapeutics Committee. The SASH Director of Pharmacy will Chair the Pharmacy and Therapeutics Committee for TCID Meetings will be coordinated to include the Medical, Quality, Nursing Staff, and any other adjunct staff required to review and discuss medication policies, procedures, conduct the necessary reviews for safe and effective medication practices at TCID. Medication Management Policies and Related Committees: • The Director of Pharmacy or designee will attend any committee meetings which includes or relates to medication management. This includes medication error review, falls, quality meetings, pain management, IC – antibiotic stewardship, risk management and/or any other TCID required medication management committees or meetings. The Director of Pharmacy will participate in the review of all policies having to do with medication management at TCID. Pharmacy Standard Operating Procedures (SOP): • The Pharmacy SOP will serve as one of the documents as required by law. Reference Materials: • The Pharmacy will maintain the necessary reference materials for the Pharmacy as required by pharmacy law.
Pharmacy and Therapeutics Committee. The Pharmacy Benefit Manager/BCBSNE’s panel of Physicians, pharmacists and other health care professionals who are responsible for pharmacy management activities such as managing and updating the Formulary. Preauthorization: The process of obtaining authorization from BCBSNE or the Pharmacy Benefit Manager for specified medications or specified quantities of medications.
