New Jersey Health Benefits Plan. At the option of each eligible full-time, permanent employee whose regular work week is twenty (20) hours or more, the Board of Education agrees to provide hospitalization insurance which includes traditional coverage, preferred provider organization and health maintenance organization through New Jersey State Health Benefits Plan as exists or as modified by the State Health Benefit Program (or and substantially equal health benefit plan), including any changes in co-pays or deductibles that may be implemented by the State Health Benefits Program for all employees and eligible dependents covered by this Agreement. In order for an employee hired after May 21, 2010 to be eligible for coverage, he or she will be required to work not less than twenty-five (25) hours per week.

New Jersey Health Benefits Plan. At the option of each full-time, permanent employee whose regular work week is twenty (20) hours or more, the Board of Education shall furnish individual, employee/spouse, parent/child and/or family, full premium coverage for all eligible employees for hospitalization, dental, prescription and vision plans during each year of this agreement. the coverage provided under hospitalization shall be equivalent to the New Jersey State Health Benefits Plan. Should the Board elect to change the health insurance carrier during the term of this agreement, the level of benefits shall not be decreased except as set forth in this contract. The Board to pay the full premium costs for such insurance for the duration of this contract. The parties acknowledge that the Board currently provides health insurance benefits pursuant to a plan with Aetna/US Healthcare. The Board currently provides two plans, the Patriot V and Patriot X. Effective the third year of this contract, Patriot V Plan is amended as follows: The co-pay for specialty care for office visits, diagnostic out-patient testing and physical, occupational, and speech therapy is increased to $15.00. The Emergency room co-pay is increased to $50.00. The maternity co- pay, first ob visit is increased to $15.00. The mental health out-patient co-pay is increased to a $25.00 co-pay and the maximum number of visits is increased to 30 visits. Substance abuse out-patient rehabilitation is increased to $15.00 co-pay with a maximum of 60 visits. Preventative care, routine eye examination (per benefit schedule) is increased to $15.00 co- pay. Preventative care, routine mammography is increased to $15.00 co-pay. Preventative care, routine gyn exam is increased to $15.00 co-pay. Chiropractic care is increased to $15.00 co- pay with unlimited visits.

  • Retiree Health Benefits 1. There is currently in effect a retiree health benefit program for retired members of LACERS under LAAC Division 4, Chapter 11. All covered employees who are members of LACERS, regardless of retirement tier, shall contribute to LACERS four percent (4%) of their pre-tax compensation earnable toward vested retiree health benefits as provided by this program. The retiree health benefit available under this program is a vested benefit for all covered employees who make this contribution, including employees enrolled in LACERS Tier 3.

  • Requiring Health Benefits for Covered Employees Contractor agrees to comply fully with and be bound by all of the provisions of the Health Care Accountability Ordinance (HCAO), as set forth in San Francisco Administrative Code Chapter 12Q, including the remedies provided, and implementing regulations, as the same may be amended from time to time. The provisions of section 12Q.5.1 of Chapter 12Q are incorporated by reference and made a part of this Agreement as though fully set forth herein. The text of the HCAO is available on the web at xxx.xxxxx.xxx/xxxx. Capitalized terms used in this Section and not defined in this Agreement shall have the meanings assigned to such terms in Chapter 12Q.

  • Workplace Safety Insurance Benefits (WSIB) Top Up Benefits If the employee is in a class of employees that, on August 31, 2012, was entitled to use unused sick leave credits for the purpose of topping up benefits received under the Workplace Safety and Insurance Act, 1997;

  • Extended Health Benefits The extended health benefits coverage for CUPE and Fire will be amended to include:

  • Health & Welfare Benefits Executive shall be eligible to participate in all health and welfare benefits provided generally to other employees of the Company.

  • Family and Medical Leave Act All employees who worked for the Employer for a minimum of twelve (12) months and worked at least 1250 hours during the past twelve (12) months are eligible for unpaid leave as set forth in the Family and Medical Leave Act of 1993. Eligible employees are entitled to up to a total of 12 weeks of unpaid leave during any twelve (12) month period for the following reasons:

  • Retiree Health Insurance Retired members of the Department receiving, or to receive City of Lincoln monthly pension checks, may participate in the group comprehensive health care plan for active City employees, provided that each retiree so desiring will execute the required forms in a timely fashion, and further provided that each retiree will be required to pay the full monthly cost at the current rates subject to any rate increases which may occur from time to time. Such payment will be made by payroll deduction from pension checks, or by direct payment in the case of an early retiree.

  • HEALTH AND WELFARE PLAN 16.01 The Employer agrees to pay the amount as set out in the Wage Schedules for all hours worked for each employee towards the Insurance Plan administered by the CLAC Health and Welfare Trust Fund.
