Vision Plans. The University shall provide two (2) vision plans. The first vision plan is Plan A, which provides coverage for an eye examination with lenses if needed, every twelve (12) months and new frames provided every twenty-four (24) months. The University will also offer a Plan B option. However, the Plan B option only offers an exam, lenses and frames every two (2) years. Employees shall be eligible for enrollment in either plan on the first of the month following thirty (30) days of employment. Eligible employees may enroll in these plans when first eligible or during an annual open enrollment period. The University shall pay the full cost per year to provide this coverage for each enrolled employee and their eligible dependents for the duration of this contract. Additionally, Plan A offers an MD rider for certain subsidized services from a panel of licensed ophthalmologists. 202. Subject to the provision contained in this Article XXV, Article XXVI, Article XXVII, Article XX, VIII, and Article XXXIX and Article III, medical, dental and vision coverages are available to employees, their dependents and others, subject to the terms and conditions contained in the University’s respective third- party insurance plan contracts, including without limitation those terms and conditions applicable to eligibility, coverage, preconditions and administration. The University will impute income, withhold taxes and otherwise account for the provision of all medical, dental and vision insurance coverage as required by federal or state law or regulation, or the decision of any court of competent jurisdiction or administrative agency having jurisdiction.
Vision Plans. The employee share of premiums (or premium equivalents) for the vision plan that the employee elects shall be 50%.
Vision Plans. Vision plans are for routine visits where you do not have any medical issues, problems, or diagnoses. We participate with VSP, EyeMed, ▇▇▇▇▇ Vision, March Vision, Avesis, Superior Vision and National Vision Administrators (NVA) Vision plans. Please inform us if you have a vision plan and the name of the plan BEFORE your exam. Most vision insurance plans include the following benefits either annually or every two years: • Annual eye exams: May not cover dilation portion of exam if required by doctor. • Eyeglass frames: May be purchased at a discount depending on plan benefits. • Eyeglass lenses: May be purchased at a discount depending on plan benefits. • Contact lenses: May be purchased at a discount depending on plan benefits. Many vision plans do not cover annual contact lens evaluations or contact lens fittings for first time wearers or established wearers that need to switch to a new brand.
Vision Plans. The employer pays the full premiums for full time employees. The employer contribution is reduced by the above amounts for employees working less than full time.
Vision Plans. A. The Vision 24 Plan and the Vision 12 Plan currently offered as options for all employees will be discontinued and no longer available as options after December 31, 2018. The current weekly contribution formula in effect immediately prior to the effective date of the Agreement shall remain in effect through December 31, 2018.
B. Effective January 1, 2019, the Vision Core and the Vision Plan Enhanced will be offered to employees on the “same basis as” offered to non-bargaining unit employees.
(1) Effective January 1, 2019, the Company will pay 100% of the premium cost of the Vision Plan Core. If the employee selects Vision Plan Enhanced, the employee is responsible for any additional premium costs between the selected plan and the Vision Plan Core.
(2) “Same basis as” is understood to mean that any improvements, modifications, reductions, eliminations or changes to the plan(s) for non- bargaining unit employees shall be automatically applicable to bargaining unit employees covered by this Agreement. Same basis applies to any and all aspects of the plan(s) including but not limited to eligibility, plan offerings, effective dates and plan designs.
Vision Plans. The Company will offer to the employees to which this Agreement applies the option to elect vision care coverage from either the Vision 24 Plan or the Vision 12 Plan for which the Company has contracted. The cost of the Vision 24 Plan shall be entirely company paid. The cost of the Vision 12 Plan shall be paid by the employee to the extent that the cost of such elected Vision 12 Plan exceeds the company contribution for the Vision 24 Plan under this Agreement based on either single or family coverage, whichever is applicable to the employee electing such Vision 12 Plan. The cost of the Vision 24 and Vision 12 Plans shall be established annually as of 1 January of each year of the Agreement based on past and estimated future experience as determined in accordance with accepted actuarial principles. This allowable Company cost shall be applicable for the ensuing twelve (12) months until the next annual review. Effective 1 January 2013, eEmployees will have coverage level options of Employee Only, Employee + 1 or Employee + 2 or More.
