Naturopath. A person who practices naturopathy, that is, a drugless system of therapy making use of physical forces such as air, light, water, heat, and massage. NOTE: Services of a naturopath are not cov- ered by XXXXXXX. NAVCARE clinics. Contractor owned, staffed, and operated primary clinics exclusively serving uniformed services beneficiaries pursuant to contracts awarded by a Military Department. No-fault insurance. A hospital or other authorized institutional pro- vider, a physician or other authorized individual professional provider, or other authorized provider that fur- nished medical services or supplies to a CHAMPUS beneficiary, but who did not agree on the CHAMPUS claim form to participate or to accept the CHAMPUS-determined allowable cost or charge as the total charge for the services. A nonparticipating provider looks to the beneficiary or sponsor for payment of his or her charge, not CHAMPUS. In such cases, CHAMPUS pays the beneficiary or sponsor, not the provider. North Atlantic Treaty Organization (NATO) member. A military member of an armed force of a foreign NATO na- tion who is on active duty and who, in connection with official duties, is sta- tioned in or passing through the United States. The foreign NATO nations are Belgium, Canada, Denmark, France, Federal Republic of Germany, Greece, Iceland, Italy, Luxembourg, the Neth- erlands, Norway, Portugal, Spain, Tur- key, and the United Kingdom. Naturopath. Out-of-hospital services of a psychologist up to a maximum of in any calendar year; Services of a legally licensed masseur up to a maximum of in any calendar year; Services of a speech therapist for correction of speech impairments up to a maximum of in any calendar year; Orthopaedic shoes up to a maximum of in any calendar year; Hearing aids prescribed by an Ear, Nose and Throat Specialist up to a maximum of in any 5-year period. Expenses incurred out-of-province are covered if benefits would have been payable had they been incurred in the employee's home province and if: an eligible employee or dependent is temporarily out-of-province on business or vacation or for furthering education and the expenses arise as a result of an emergency or unexpected sudden illness, or the required medical treatment is not readily available in the employee's home province. If medical treatment is readily available elsewhere in Canada but the employee seeks treatment outside Canada, benefits will be limited to the reasonable and customary charges of the nearest Canadian medical centre equipped to provide the necessary treatment. The following expenses are excluded: Services not listed as covered expenses; Delivery and transportation charges; Services and supplies which are required for recreation or sports but which are not medically necessary for regular activities; Services and supplies received during a period of hospital confinement which began before the employee became eligible for insurance coverages.   • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).

  • Hospice g. Individuals whose permanent residence and principal work location are outside the State of Minnesota and outside of the service areas of the health plans participating in Advantage.
