Group Life Insurance Coverage. For a twenty-four (24) month period, after the Executive’s CIC Termination or a twelve (12) month period after the Executive’s Non-CIC Termination, as applicable, the Company shall continue Executive’s basic life insurance coverage. Executive will be entitled to the life insurance conversion rights required by applicable law.

Group Life Insurance Coverage. The Employer agrees to pay for each full-time regular employee Group Life Insurance according to the following schedule: Group I - $2.500 Life Insurance Coverage Upon the completion of three (3) years of regular full-time service, an employee shall not be required to make monthly group life insurance payments up to $2,500 of coverage. Group II - $2.500 up to $5.000 Life Insurance Coverage Upon the completion of five (5) years of regular full-time service, an employee shall not be required to make monthly group life insurance payments up to $5,000 of coverage. Group III - $5.000 up to $10,000 Life Insurance Coverage Upon the completion of ten (10) years of regular full-time service, an employee shall not be required to make monthly group life insurance payments up to $10,000 of coverage. Group Life Insurance Coverage. The Company shall pay to the Executive a monthly cash payment during the Welfare Benefit Continuation Period equal to the monthly premium amount (as determined herein) applicable for the Executive to continue to receive the same basic level of life insurance coverage in effect for Executive under the Company’s group life insurance plans at the time of Executive’s employment termination. The amount of such monthly premium shall be communicated by the applicable insurance carrier to the Company within thirty (30) days following the Executive’s employment termination, provided that if no such amount is communicated, the amount of such monthly premium shall be equal to the applicable monthly premium amount in effect for such coverage under the Company’s group life insurance plans at the time of Executive’s employment termination. The Executive will be entitled to the life insurance conversion rights required by applicable law at the end of such period.
Group Life Insurance Coverage. The Employer will provide each regular employee hired before October 1, 2013 and working on a regular work schedu1e of not less than Twenty (20) hours per week, and each regular employee hired after October 1, 2013 and working on a predetermined work schedu1e of not less than Thirty (30) hours per week, with a group life insurance policy in an amount equal to one times the employee's budgeted straight time annual earnings including double indemnity for accidental death and dismemberment. The Employer will pay the premiums for such coverage. This coverage will be effective on the first day of the month following completion of ninety (90) days of continuous employment as a regular employee.
Group Life Insurance Coverage. The City shall maintain and provide full-time employees only with a fully paid group term life insurance policy. The amount of the term life insurance to be provided each employee shall be in an amount that is equal to the next even $10,000 above the employee’s annual base salary, not to exceed $70,000 coverage.
