Eligibility for Insurance. 1. Any regularly contracted employee who works at least thirty (30) hours per week shall be entitled to have the Board pay one-hundred percent (100%) of the Board’s total contribution for all of the types of insurance listed in the following paragraphs.
Eligibility for Insurance. The employer shall provide insurance to employees of the district working twenty-one (21) or more hours per week as of October 27, 1992. The employer shall contribute 90% for eligible employees toward the purchase of Plan A. The balance of the insurance costs shall be paid by payroll deduction.
Eligibility for Insurance. Probationary employees shall be eligible for insurance benefits pursuant to the terms of the insurance policy.

Extra Accident Insurance is provided while the employee is insured for Life Insurance during active service and while Life Insurance is continued during layoff or leave of absence, as specified in Article III, Section 2, and during periods of total disability as set forth in Article III, Section 3, but not beyond age 68 in any event.

Bargaining unit members employed prior to October 1, 1983, for fifty percent (50%) or more of the regularly defined daily or weekly working hours shall be eligible to participate in the District group insurance programs, and shall be entitled to all District insurance benefits for the first day of the first month following the first day of their employment. Bargaining unit members employed on or after October 1, 1983, shall be eligible to participate in the District's group insurance programs if regularly assigned to work fifty percent (50%) or more of the Regularly defined daily or weekly working hours. The District contribution shall be pro-rated as follows: 50% assignment - Not to exceed 50% contribution. 75% assignment - Not to exceed 75% contribution. If the bargaining unit member's working hours are reduced at the request of the District, the District's contribution remains the same.
Eligibility for Insurance. A. To be eligible for insurance under the Group Policy, you must be an Eligible Person. An Eligible Person is an Employee who has met the following requirements:
Eligibility for Insurance. 16.2.1 Unit Members Employed Prior to December 31, 2002 All unit members who are regularly assigned to work at least six (6) hours per day shall be entitled to coverage under the programs provided for in Article 16.1 above. Employees who are regularly assigned to work less than six (6) hours per day may enroll in the various insurance programs, consistent with JPA regulations, and have a prorata of their premiums paid in the same proportion as their daily assigned work hours bears to eight (8). Employees regularly assigned to work fewer than three (3) hours per day shall bear their full premium costs.
Eligibility for Insurance. Employees of the Employer shall be eligible for insurance from the date as outlined in the Waiting Period mentioned in the Schedule, provided they are Actively at Work at such date. The Employee’s Dependents shall be eligible for insurance on the same date as the Employee himself or on a subsequent date on which they become Dependents of the Employee. Employees not Actively at Work on the date on which they would otherwise be eligible for insurance shall become eligible for insurance on the first (1st) day of the Month following the date of their return to work in the capacity for which they are made eligible for insurance. Insured Persons must be full-time residents of Canada and be covered under their Provincial Government Health care plans. In the event that two (2) Insured Employees are Spouses of each other and eligible for insurance under this policy, one (1) of the two (2) may choose to be insured as an Insured Spouse of the other in which case they will not be considered to be an Insured Employee or both may choose to be covered as an Insured Employee in which case neither will be eligible as a Spouse. Notwithstanding preceding paragraphs, Employees who are Actively at Work on the Master Policy Effective Date whose coverage under another group insurance policy terminates on such date shall be eligible for insurance on the date on which the present policy shall come into force.
Eligibility for Insurance. Adjunct and part-time faculty who teach a minimum of 40 contact hours of instruction in or more courses during an Academic Year (Fall, Winter, Spring or Summer) or a minimum of 75 hours of individual instruction or tutoring in a semester in an Academic Year (Fall, Winter, Spring or Summer) (Eligibility Requirements) may apply for health insurance coverage through the United Healthcare Point-of-Service Plans (Value or Advantage Plans), or their reasonable equivalents in which the University, from time to time, may participate and which is available to the University’s full-time faculty. Such health insurance will be effective in the calendar year following the Academic Year during which the Eligibility Requirements were met.
Eligibility for Insurance. Employees to be eligible for enrollment for insurance programs or benefits must work a minimum of fifteen (15) hours per week, as restricted for eligibility by the insurance carriers, and any employee who is concurrently working for another employer, where insurances are provided, shall not be eligible for insurance programs or benefits from the School District which are provided by another employer.
