Additional documentation in the case file should include the best interests factors considered, participants involved in the collaborative process, and the school placement decision.

Documentation and Notification. It is my responsibility to supply Koda with a list of my existing works and to supplement this list with informa- tion of my new works as soon as they have been created. I furthermore agree to supply Koda with all the informa- tion of my works necessary to Koda in connection with collection, registration and distribution. As a part of the process, a Staff Incident Report or another form of documentation of the violation will be completed by the on-site AMIDEAST Education Abroad staff, the Student(s) involved, and/or any other involved parties. As appropriate, AMIDEAST Headquarters staff may conduct similar processes to understand misconduct or violations of policies independent of on-site staff. If appropriate, the Student will be asked to prove pertinent information, including any written statements from witnesses, regarding the incident. In the event that a Staff Incident Report is not completed, the Student Agreement and Release remains in effect and is not modified in any way. Upon completion, the Staff Incident Report or other form of documentation of the violation will be sent electronically to AMIDEAST Headquarters in Washington, D.C., and will be added to the Student(s) file. AMIDEAST, at its discretion, may send documentation related to the violation to the Student’s home institution, if currently enrolled. The Student will be notified verbally or in writing by an AMIDEAST staff member of any alleged misconduct or violation(s) of this Student Agreement and Release. However, when the offense is serious enough to warrant dismissal, no warning shall be necessary. Administrative Review Process The on-site AMIDEAST Education Abroad staff, the Country Director (or designee), and AMIDEAST’s U.S.-based staff will make a decision based on the available information regarding the consequences for violating the Student Agreement and Release. Disciplinary action to be taken will be determined during this process. The Student will be notified in writing, as soon as possible, of AMIDEAST’s decision regarding disciplinary actions resulting from the violations of AMIDEAST policies. Any disciplinary actions imposed for violating the Student Agreement and Release are effective immediately. In the case of serious misconduct or for safety concerns, the on-site AMIDEAST Education Abroad staff and Country Director (or designee) has the sole discretion and authority to immediately relocate any Student to a different place of residence or dismiss the Stud...

  • SECURITY POLICIES AND NOTIFICATIONS State Security Policies and Procedures The Contractor and its personnel shall review and be familiar with all State security policies, procedures and directives currently existing or implemented during the term of the Contract, including ITS Policy NYS-P03-002 Information Security Policy (or successor policy). Security Incidents Contractor shall address any Security Incidents in the manner prescribed in ITS Policy NYS-P03-002 Information Security Policy (or successor policy), including the New York State Cyber Incident Reporting Procedures incorporated therein or in such successor policy.

  • COMPLIANCE WITH BREACH NOTIFICATION AND DATA SECURITY LAWS Contractor shall comply with the provisions of the New York State Information Security Breach and Notification Act (General Business Law § 899-aa and State Technology Law § 208) and commencing March 21, 2020 shall also comply with General Business Law § 899-bb.
