Long-term services and supports definition

Long-term services and supports means the Medicaid services and supports provided under a CMS approved waiver to assist individual's needs and to avoid institutionalization as defined in OAR chapter 411, division 4 and defined as Medicaid Home and Community-Based Settings and Services (HCBS) and as outlined in OAR chapter 410, division 172 (Medicaid Payment for Behavioral Health Services).

  • Grantee shall act upon at least 70.0% of referrals within 15 calendar days of receipt from the Long-Term Services and Supports (LTSS) Screen.

  • Coordination of Care means, for the purposes of this Agreement, the MA Dual SNP’s mechanisms that promote increased coordination between the services provided by the MA Dual SNP and the Long-Term Services and Supports (“LTSS”) the Member receives from Medicaid, including but not limited to discharge planning, disease management, and care management.

Long-term services and supports or “LTSS” means all of the following:
