SHOES AND BOOTS. The City agrees to furnish and maintain the required uniforms of uniformed employees as in the past. Replacement of uniforms, boots, shoes and raincoats, etc., shall be on the salvage system, and old articles shall be turned in to the person designated by the director in order to receive replacement of these articles of clothing. The combined boot and shoe allowance shall be $200.00 every two (2) years. Employees shall present either a xxxx for payment to the supplier or a receipt for reimbursement to the employer. SHOES AND BOOTS. Must be clean and like new. No scuffed toes. Shoes should be joined together with large bag tie or string. Toys & Games: Must be in excellent working condition and include all pieces. Toys requiring batteries must have working batteries. Please place all loose pieces in zip-lock bag and attach to toy or game. Puzzles: Must include all pieces. If in a box, tape shut; if a wooden puzzle, secure in a plastic bag with label taped to outside. Books: Place tag inside cover with removable tape. For several books, tie with string or use rubber bands and attach price on top book. Sports Equipment & Dance Wear: Bikes, roller blades, scooters, skates, tennis rackets, bats, balls, gloves, cleats, karate uniforms, etc. Dance shoes, leotards, dance costumes, gymnastic attire, etc. All must be in good condition. Car Seats: Can be accepted if it is currently before their expiration date. This date is located on the plastic part of the car seat. Please write that date on the front of you car seat's consignor tag. All seat covers and straps must be thoroughly cleaned. Items NOT accepted: Clothing not in season. At our Fall sale we accept Fall & Winter clothing. At our Spring sale we accept Spring and Summer clothing. No advertising tees such as YMCA tees or those from your last vacation spot. NO stuffed animals unless they are battery operated. No previously worn underwear. Only new underwear with tags or those in original bag. No recalled or unsafe items. No baby equipment manufactured before 2007. No cribs.

SHOES AND BOOTS. This is a thirty-six (36) month contract. Prices shall be firm as offered for the first twelve (12) months of the contract. Contractor may adjust prices once a year only. Said adjustment shall be based on the “Producer Price Indexes” table for Commodity Groupings and Individual Items, sub-category “Footwear Code 043", published by the U.S. Department of Labor - Bureau of Labor Statistics. The Contractor must give the Purchaser a written notice with a sample of the calculation and a copy of the “Producer Price Indexes” table for Commodity Groupings and Individual Items, sub-category “Footwear Code 043". SPECIAL CONDITIONS (Continued) The bid price in the proposal shall be adjusted as follows: The bid price is subject to a price adjustment effective on the anniversary date of the start of the contract, provided the vendor gives the City a thirty (30) day written notice. This increase or decrease in the base price shall be in proportion to the percent (%) change (increase or decrease) in the latest unadjusted wholesale price in the above referenced indices. Subsequent annual adjustment shall be calculated based on the index used in the previous adjustment and the index three (3) months prior to the month of adjustment.

Contracting Officer must receive Purchaser’s written request for a market-related contract term addition before the expiration of this contract. No more than 3 years shall be added to a contract's term by market-related contract term addition unless the following conditions are met:
