RESPONSIBILITIES & REQUIREMENTS. The board resolution, order, motion, ordinance or similar document shall be approved by the State before the parties can enter into a valid contract. The Contractor shall not perform any tasks prior to contract execution. A list of all tasks and deliverables are set forth below. Administrative Requirement - Board Resolution Each county is required to have a Board legally binding resolution, order, motions or ordinance or similar document from the local governing body authorizing execution of the agreement.

Requirements for services provided include, but are not limited to, the following: • Contractor shall rent two (2) 12-stall shower trailers to the state • Contractor shall deliver the trailers to the job site, and remove them from the site at the completion of the agreement • Contractor shall provide all cables, hoses, pumps, fittings, and labor required for setup and breakdown of shower trailers • Contractor shall rent two (2) Shower/Restroom combo trailers to the state. These trailers have two full ADA-compliant stalls with shower, sink and toilets.

Requirements for services provided include, but are not limited to, the following: • Contractor shall ensure all updated CDC or County guidance are implemented as appropriate. • Contractor shall comply with all Cal/OSHA and Federal OSHA requirements specific to worker safety. • Contractor shall supply meal preparation and delivery service to support the Santa Xxxxx Federal Medical Station. • Contractor shall rent and keep in working order refrigerators, electric hot boxes and cables for the duration of the contract, as listed in Exhibit B, Attachment 1. • The meal preparation and delivery services shall include breakfast, coffee & tea service (2), bottled water service (2), snack lunch bag, snack, disposable cutlery & napkin packets, dinner, and a late-night snack bag daily for the duration of this contract, as described in Exhibit B, Attachment 1. This service shall be for up to 350 people. • Contractor shall ensure workstation is neat, clean and sanitary. Contractor is responsible for their own supplies to perform this function. • Contractor shall ensure all trash and recycling is disposed of in a proper manner.
