PERMISSION TO PARTICIPATE. I, the parent/guardian of the above-named participant hereby acknowledge that my child is in good general health and I give my approval for my child to participate in any and all Pop Warner national, regional, league/conference, association and team/squad activities, including transportation to and from the activities by a licensed driver with proof of insurance. . I understand, hereby give my approval for, and assume any and all risk of my child's use of various playing surfaces and conditions, including, but not limited to, dry and wet natural and artificial grass, hard dirt, and/or mud and I hereby acknowledge and understand that said surfaces may be regular or very irregular.
PERMISSION TO PARTICIPATE. I certify that I (i) am the parent or legal guardian of the child (“Participant”) being allowed to participate in the NFL FLAG football program (the “Program”), as part of a NFL FLAG league administered by a local league organizer; (ii) am of legal age and am freely signing this Player Registration Agreement without any inducement or assurance of any nature; and (iii) have read this form and understand that, by signing this form, I may be giving up certain legal rights and remedies. I agree that the terms of this Player Registration Agreement are binding on both me and the Participant.
PERMISSION TO PARTICIPATE. Medical Release - One signature is required. The undersigned, parent(s) or legal guardian(s) of certify that he/she is of good physical condition and is fit for participation in the activities of California Dolphin Swim Team. I/We understand these activities include aerobic exercises, swim workouts, swim meets, and other activities routinely associated with the development and participation in USA Swimming functions (activities may include transportation to and from meets and swim related social functions). The undersigned shall jointly and independently hold California Dolphin Swim Team, all officers, agents, and employees of California Dolphin Swim Team harmless from any and all liabilities for personal injury and property damage which might arise out of or relate to the conduct of participation in the activities of California Dolphin Swim Team. I/We fully understand the risks associated with physical activities such as competitive swimming and hereby give my/our permission for participation to the above participant for whom we are/I am the legal parent(s) or guardian(s). I/We also hereby agree to the provision of emergency medical procedures that may be required due to illness or injury which might arise out of the participation in the activities with California Dolphin Swim Team to provide emergency medical treatment through a fully licensed hospital or through the family physician or dentist listed. I/We authorize transportation of my/our child by ambulance in an emergency situation. Further, I/We agree to pay all costs associated with such medical care and emergency transportation. PERMISSION TO PARTICIPATE. I hereby give consent for my child to participate in a program conducted by West Bloomfield Parks.
PERMISSION TO PARTICIPATE. I hereby grant permission for my child to participate in any and all of the school activities and to use the play equipment to include all indoor and outdoor toys and equipment. I give my permission for field trips requiring transportation in a chartered vehicle and must sign an additional and specific permission slip for my child to attend an event, function, or to leave the premises on a chartered vehicle. I reserve the right to decline my child’s participation in any excursion off the premises.
PERMISSION TO PARTICIPATE. I/We hereby authorize and consent to our child’s participation in interscholastic athletics and sports. We understand that the sport(s) in which our child will be participating is potentially dangerous, and that physical injuries may occur to our child requiring emergency medical care and treatment. I/We assume the risk of injury to our child that may occur in an athletic activity. Will Not Hold Responsible In consideration of the acceptance of our child by the Lionville Middle School in its athletic program, and the benefits derived by our child from participation I/We agree to release and hold harmless the Downingtown Area School District/Lionville Middle School, its members, the Superintendent of schools, the principal, coaches, athletic directors, and any and all other agents, servants, and/or employees and agree to indemnify each of them from any claims, costs, suits, actions, judgment, and expenses arising from our child’s participation in interscholastic athletics.

I certify that I (i) am the parent or legal guardian of the child (“Participant”) being allowed to participate in the National Hockey League branded street/ball hockey program known as NHL STREET, including regional and national tournament(s) (the “Program”), as part of a NHL STREET league administered by a local league organizer; (ii) am of legal age and am freely signing this Player Registration Agreement without any inducement or assurance of any nature; and (iii) have read this Player Registration Agreement and understand that, by signing this Player Registration Agreement, I am giving up certain legal rights and remedies on behalf of myself and the Participant. I agree that the terms and conditions of this Player Registration Agreement are binding on both me and the Participant.
PERMISSION TO PARTICIPATE. Having reviewed, understood and agreed to the foregoing terms and conditions, the undersigned grants unrestricted permission for the Named Player to participate on XX Xxxxxxx and to participate in all training sessions, games, and other activities. This permission extends to any travel to and from any and all training sessions, games, and other activities arranged by XX Xxxxxxx.
PERMISSION TO PARTICIPATE. Having reviewed, understood and agreed to the foregoing terms and conditions, the undersigned Parent grants unrestricted Permission for the named Player to participate in SOUND FC and Team and to participate in all practice sessions, games, and other activities involving SOUND FC and/or the Team during the seasonal year. This permission extends to any travel to and from any and all practice sessions, games, and other activities sponsored and arranged by SOUND FC.
