The OUTCOME OBJECTIVES clause defines the specific results or goals that the parties intend to achieve through their agreement. By clearly stating these objectives, the clause ensures both parties have a shared understanding of expectations and provides a basis for evaluating performance and accountability.
OUTCOME OBJECTIVES. 1. For completed episodes, there will be no more than ten percent (10%) recidivism of clients within thirty (30) days following discharge.
2. For completed episodes, Contractor will maintain or reduce the 2009- 2010 combined average length of stay. Contractor will make best efforts to meet such goals, but failure to meet any or all of these goals does not constitute a breach, material, or otherwise of the Agreement. County's payment obligation is in no way contingent on meeting any of the stated goals.
OUTCOME OBJECTIVES. 1.2.1. Ninety percent (90%) of clients with a planned discharge (excluding clients who elope and do not return or who die) have improved their functioning
1.2.2. At least 90% of residents admitted will complete six months of residency or be successfully placed at a lower level of care. Patients who are discharged and readmitted within three weeks will be considered to have continuous residency.
1.2.3. For patients completing three months of residency, acute psychiatric hospitalization will be reduced 70% in the six months following discharge compared to the average six-month period in the two years prior to admission.
OUTCOME OBJECTIVES. The Agency will monitor the services provided by the Contractor under this training program to evaluate the effectiveness and quality of the services received. To measure whether or not staff are able to put into practice the new skills, concepts and knowledge provided by this training program, the Contractor will evaluate how useful staff felt training was to their practice and secondly to evaluate staff transfer of learning. Participant evaluations of the usefulness of the training completed at the end of the training: ▪ For mandatory training a minimum of 70% of the respondents shall rate the overall usefulness as very good to excellent ▪ For non mandatory training a minimum of 80% of the respondents shall rate the overall usefulness as very good to excellent. Follow up evaluations of 60% of training participants to assess the transfer of learning: ▪ A minimum of 80 %of the respondents shall indicate that their knowledge increased as a result of the training. ▪ A minimum of 70 % of respondents shall identify at least two skills, tools, concepts, knowledge or policies and procedures that they will use in the workplace.
OUTCOME OBJECTIVES. 2.1 CONTRACTOR shall provide the following number of direct service hours from July 1st through June 30th, for each year of the Contract:
2.1.1 Minimum of 13,000 hours of direct services in Year One (1) of the Contract.
2.1.2 Minimum of 13,075 hours of direct services in Year Two (2) of the Contract.
2.1.3 Minimum of 13,160 hours of direct services in Year Three (3) of the Contract.

OUTCOME OBJECTIVES. ADMINISTRATOR, at its sole discretion, may require changes to the Outcome Objectives listed below.
3.1 Throughout the term of the Contract, CONTRACTOR shall measure progress through the following objectives:
3.1.1 A minimum of forty percent (40%) of PARTICIPANTS shall complete a Psychosocial Assessment.
3.1.2 A minimum of eighty percent (80%) of PARTICIPANTS who completed a Psychosocial Assessment will develop an Individualized Safety Plan using a Harm Reduction Model.
3.1.3 A minimum of twenty-five percent (25%) of PARTICIPANTS who DocuSign Envelope ID: 40A3DBA1-1EC3-47FB-9184-F7D865CC9EEA completed a Psychosocial Assessment shall engage in a minimum of one (1) additional Support Service within thirty (30) days of initial contact with Resource and Support Services for CSEC.
OUTCOME OBJECTIVES. 3.1 CONTRACTOR shall meet the following performance goals for PSEP:
3.1.1 Seventy-five percent (75%) of enrolled PARTICIPANTS shall be placed in employer worksites.
3.1.2 Thirty percent (30%) of all placements will be at public or non-profit sector worksites.
3.1.3 Sixty percent (60%) of PARTICIPANTS will obtain unsubsidized employment within sixty (60) days of completing PSEP placement. CONTRACTOR and ADMINISTRATOR may mutually agree to extend this time frame if there is documentation of pending employment due to the hiring process.
3.2 CONTRACTOR shall meet the following performance goals for WEX:
3.2.1 Seventy-five percent (75%) of enrolled PARTICIPANTS shall be placed in worksites.
3.2.2 Eighty percent (80% ) of exiting PARTICIPANTS will obtain unsubsidized employment within thirty (30) days of completing WEX placement.
3.3 CONTRACTOR shall meet the following performance goals for VTR:

3.3.2 Sixty percent (60%) of PARTICIPANTS will obtain unsubsidized employment or subsidized employment within thirty (30) days of completing VTR program. CONTRACTOR and ADMINISTRATOR may mutually agree to extend to sixty (60) days when certification is delayed or pending employment due to hiring process.

3.2 CONTRACTOR shall meet and report out on a monthly basis the following annual performance goals during the term of this Agreement:
3.2.1 CONTRACTOR shall attempt to contact one hundred percent (100%) of referred PARTICIPANTS within three (3) business days of referral receipt.
3.2.2 CONTRACTOR shall make a minimum of three (3) contact attempts, on three (3) varying days and times, via PARTICIPANT’s preferred method of communication (e.g., text, phone call, email) within three (3) business days, after the initial contact is unsuccessful, for one hundred percent (100%) of these PARTICIPANTS.
OUTCOME OBJECTIVES. 9 8.3.1 One hundred percent (100%) of participants will be provided Employment 10 Readiness training.
11 8.3.2 One hundred percent (100%) of participants will receive at least three (3) 12 hours of instruction on how to clean an apartment.
13 8.3.3 One hundred percent (100%) of participants will have a housing transition 14 plan upon graduation from the THPP.
15 8.3.4 One hundred percent (100%) of participants will receive financial 16 management training.
17 8.3.5 One hundred percent (100%) of participants will be provided instruction on 18 meal preparation.
19 8.3.6 One hundred percent (100%) of eligible participants will have a state issued 20 identification card if they are eligible.
21 8.3.7 One hundred percent (100%) of participants will be provided tutoring upon 22 acceptance of tutoring services if it is necessary for the completion of high school or equivalent.
23 8.3.8 One hundred percent (100%) of participants will be provided disaster 24 preparedness and readiness training.
25 8.3.9 One hundred percent (100%) of participants will be provided relationship 26 violence awareness training.
